Anencephaly and Hydrocephalus in Monoamniotic Twins Conceived by in Vitro Fertilization-Embryo Transfer

摘要

背景：無腦兒和水腦兒都很常見，但是無腦兒及水腦兒同時發生於同卵雙胞胎卻未曾有報導過，因此，我們報告此一特殊病例。病例報告：一位35歲女性，和45歲先生結婚五年，因不孕症於一私人診所進行試管嬰兒，懷孕成功，妊娠八周時發現單一妊娠囊內有兩個胚胎；於妊娠十二周又六天時發現其中一胎兒為無腦兒；妊娠十七周時進行羊膜腔穿刺術，染色體報告為正常的46XX；妊娠十九周發現另一胎兒腦室有較正常為大的情形，且兩個胎兒的臍帶有互相糾纏的現象；於妊娠二十周證實另一胎兒為水腦兒；於是應孕婦的要求，給予終止妊娠，經陰道分娩出無腦兒及水腦兒各一，且兩個胎兒的臍帶亦有大小不一的現象。結論：妊娠早期的超音波檢查可以早期發現胎兒的異常，同卵雙胞胎有著更高的危險性，因此，早期密切仔細的超音波檢查是非常必要的。造成胎兒神經管缺損的原因是多重因素，而葉酸被認為是原因之一，本病例於試管嬰兒進行過程中雖然有給予補充葉酸，但仍不幸發生胎兒神經管缺損的情形，或許平時未懷孕就應多攝取葉酸，或許還有其他許多未明白的因素吧。

Anencephaly is a common human malformation that occurs in 1.4-4.7/10000 deliveries worldwide. Discordant anencephaly occurs with greater frequency in monoamniotic twins than in singleton pregnancies. We present a case of monoamniotic twins after in vitro fertilization-embryo transfer, in which one of the twins had anencephaly and the co-twin had hydrocephalus. The anencephaly was found at the 10(superscript th) week of gestational age, and the hydrocephalus was diagnosed at the 19(superscript th) week. The pregnancy was terminated at the 20th week of gestational age.
