照顧肌萎縮性側索硬化症病人接受氣切之護理經驗一個案報告
Nursing Experience of an Amyotrophic Lateral Sclerosis Patient who Underwent Tracheostomy: A Case Report
摘要
本篇係描述一位罹患肌萎縮性側索硬化症之病人，因病情快速惡化，導致呼吸功能衰退，面臨緊急插管及氣切手術，病人心態對未來產生絕望感，而家屬也因過度擔心，照護上產生不安及緊張。護理期間為2013年2月24日至3月6日，藉由實際照護觀察、身體評估及病歷資料，與家屬會談等方式收集資料，運用Gordon十一項功能性健康型態為評估架構，確立個案主要健康問題包括：呼吸道清除功能失效、無望感及照顧者角色緊張等。於護理過程中運用叩擊、震顫技巧、兩小時變換姿勢及協助按摩等方式，減輕個案身體不適感，並配合定時抽痰維持呼吸道通暢。除醫療上的必要專業作業外，另引導個案與家屬表達出內心感受，透過適時陪伴建立彼此信任感。並指導個案及其家屬，對疾病有正確的認知及學習各項照護技巧，透過醫療團隊共同照護，在個案面對全身漸漸失去功能時，能以正向態度面對，並提供完善護理品質，以發揮護理獨特功能。

This report describes the nursing experience of a patient with amyotrophic lateral sclerosis whose condition deteriorated rapidly leading to respiratory failure and facing an emergency intubation and tracheostomy. The patient was desperate and the family members were over-worried, and these caused anxious and tension in medical care. The nursing care period was from February 24 to March 6, 2013. Data was collected by observations, physical assessments and review of medical records, interview with family members, etc. Major health problems including airway clearance function failure, hopelessness and care stress were established by using the Gordon eleven functional health patterns. Percussion, tremor skills, changes of position every two hours and massage were used to help relieve physical discomforts of the patient, together with suction to maintain airway patency. In addition, we guided the patient and the family members to express their feelings and build mutual trust through accompanying at proper time. We also transfer correct knowledge of the disease and various caring skills to the patient and family members. Through these collaborative work, we provided quality and carried out the unique functions of nursing care, and the patient faced the deteriorating body with positive attitude.
