A Nursing Experience with a Premature Infant of Neonatal Lupus Syndrome

摘要

本文主要探討一位新生兒狼瘡早產兒的護理經驗。筆者自2010年04月27日至2010年05月7日擔任主護護理師期間，運用系統性的觀察、評估與會談方法，收集個案的生理狀態、行為變化及家庭評估後，確立個案有皮膚完整性受損、無效性保護能力、混亂性嬰兒行為的健康問題；案父母方面則有照顧者角色緊張的問題。在護理過程中，筆者針對新生兒狼瘡症候群所引起的皮膚紅疹、血小板過低給予個別性的護理措施，並運用發展支持性照護的護理原則減少不必要的刺激，協助修正個案的行為。而對於案父母則主動了解其壓力源，協助透過執行袋鼠式護理及學習照顧新生兒狼瘡症候群早產兒的知識與技巧，使其及早適應父母角色。希望藉此護理經驗能提供臨床護理人員在照顧新生兒紅斑性狼瘡早產兒之參考。

This article explores the experience of caring for a premature infant with Neonatal Lupus syndrome. During the period from April 27th, 2010 to May 7th, 2010, the author worked as the main register nurse for this patient; conducted systematic observations, evaluations, and interviews; recorded the patient's physical status, behavioral changes, and family evaluation; and established the following nursing diagnoses: impaired skin integrity, ineffective protection, and disorganized infant behavior. The patient's parents had a nursing diagnosis of caregiver role strain. During the nursing care process, the main author provided individual nursing intervention focusing on skin rash and thrombocytopenia caused by Neonatal Lupus syndrome, applied developmentally supportive care to reduce unnecessary stimulation, and assisted in correcting the patient's behavior. For the parents, the author took the initiative to understand the source of their stress, assisted them in adapting to the parenting role by teaching them kangaroo care and other techniques for caring for a premature infant with Neonatal Lupus syndrome. It is hoped this experience can be a reference for clinical nurses caring for premature infants with Neonatal Lupus syndrome.
