功能性子宮出血之中醫治療病案報告

摘要

功能性子宮出血是導因於無排卵性的月經週期，或子宮收縮不良以及內膜崩落不全的疾病，現代醫學通常使用荷爾蒙療法治療，但常常無法長期有效的控制。本病屬於中醫崩漏疾患，發病過程與肝脾腎三臟密切相關，常見病機有肝氣鬱結，肝鬱化火、脾虛無法固護血液於脈道，以及腎虛封藏失職等，共通點是臟腑虛弱皆會導致衝任不固，而致經血非時而下或過期不止，遂成崩漏。本例病案報告一25歲婦女非週期性子宮出血的診治過程，詳細描述臨床症狀，病機推演與治療方法。經過三週中醫藥物治療後，病患月經停止且腹痛與脹氣等症狀均消失，且追蹤3個月均不再復發月經崩漏，希望其治療成果可作為日後功能性子宮出血患者治療參考。

Dysfunctional uterine bleeding in adult is a disease that leads to endocrine dyscrasia of adult female. Western medicine usually uses combinations of drugs with hemostasis, but it causes distinct gastrointestinal reaction and patients in the irradiated sites causing discomfort. DUB belongs to the category of metrorrhagia and metrostaxis in Traditional Chinese Medicine. The attack is associated with the liver, the spleen and the kidney .The stagnation of liver-qi which leads to fire transmission, the lack of the source of growth and development of qi and blood due to hypofunction of the spleen and the dereliction of duty storing and blood keeping caused by the kidney deficiency are responsible for the out-of-control of Chong and Ren of blood. Consequently, menses spurts out at irregular intervals, and forms metrorrhagia and metrostaxis. Here presented a treatment course of an 25-year-old female with dysfunctional uterine bleeding, the treatment course includes sophisticate description of clinical manifestation and method of treatment. Intervention in three weeks’ medicine and application of the combination of plugging flow, clarifying the source and involution, the patient's symptoms improved significantly, to achieve satisfactory results, the treatment can be provided as a reference.
