應用Peplau理論照顧一位妄想型精神分裂症病人之護理經驗

Application of Peplau's Theory in the Nursing Care of the Patient with Paranoid Type of Schizophrenia

摘要


本個案報告是運用Peplau理論,照顧一位妄想型精神分裂症個案之護理經驗。筆者透過觀察法、訪談法進行資料收集。經由評估發現個案有思考過程變異、社交互動障礙/不良、無效性因應能力、不遵從/藥物等護理問題。本文以Peplau理論為架構,透過治療性人際關係中的介紹期、認同期、探索期、解決期,漸進式與個案建立信任的護病關係,幫助個案緩解焦慮、澄清妄想內容、增進壓力因應能力。在這段關係中,護理人員扮演替代者、資源提供者、指導者、諮商者的角色,來幫助個案改善人際關係。結果顯示:運用Peplau理論於妄想型精神分裂症個案,不但幫助個案重新經驗正向人際經驗,學習與他人建立信任感,更可以協助筆者檢視自己與個案的關係,也讓筆者獲得成長。

This report describes the application of Peplau’s theory in the nursing care of the patient who was diagnosed as paranoid type schizophrenia. We have collected the information and made nursing assessment through close observation and interview, in which we’ve found many nursing problems that may alter in process, ineffective coping, impaired social interaction, and noncompliance. Based on Peplau’s theory containing four phases of orientation, identification, exploitation, and resolution, this paper implemented the-above-mentioned theoretical framework hopefully to gradually establish atherapeutic relationship with patients. In the therapeutic relationship, nurses play the role of resource person, the instructor, counselor, teacher who could help the client to ameliorate the impairment ininterpersonal relationship, to improve the stress coping skill. The results finally shows that applying Peplau’s theory in the nursing care for those who were diagnosed as paranoid type schizophrenia, can not only help the patients re-experience positive interpersonal relationship, but also help the nurse be aware of their relationship, and so do the nurse also get growth.
