論護病關係中之同理心、傳移與愛

Discussing Empathy, Transference and Love in the Nurse-Patient Relationship

摘要


一般人對護士的幻想存在著令人無法理解的兩極化現象:從理想化的護士形象,如南丁格爾,一直到性幻想中的俏護士形象。此外,若考量真實情境中的護士,那麼上述的兩極化幻想更顯示其脫離現實的特徵。就精神分析的觀點而言,幻想的功能在於慾望的表達。同樣地,人對護士的幻想也涉及了對護士的慾望。如此,顯然護士的角色就不能單純地從臨床的角度被看成只是一個提供照料的人,而是在醫病關係中,護士也捲入了更深層的要求與慾望的關係。但人對護士的慾望以及護士對該慾望的回應,卻是甚少被討論的議題。本文試圖鋪展開這些問題,以精神分析觀點探討護病現象中同理心、傳移與愛的關係,進而形就「護理」(nursing)作為一門「照護科學」(science of caring)之可能論述。

This article constructs and illustrates a discourse for nursing as a caring science from the perspective of psychoanalysis to explore the relationships of empathy, tranference, and love in the nurse-patient relationship. Many people fantasize the image of a nurse being polarized: either the ideal image of Florence Nightingale or someone dressed in a sexy nursing uniform. In real life, such polarized fantasies are far from reality. In psychoanalysis, fantasy serves to stage desires; therefore, people's fantasies for nurses involve claiming the desires for themselves. This desire for a nurse and the nurse's response to that desire is a rarely touched upon issue. In a clinical setting, a nurse should not be viewed as a mere care-provider role. Instead, a nurse being drawn into a deeper layer of relationship of being desired.
