新北市中級救護技術員在職教育課程需求評估

In-Service Educational Programs for Class II Emergency Medical Technicians in New Taipei City: A Needs Assessment

摘要


背景:119消防隊救護技術員是緊急病人後續醫療品質及預後決勝的關鍵。面對複雜的救護情境,救護技術員需在職教育以增強救護能力。目的:探討中級救護技術員在職教育課程之需求。方法:本研究採用橫斷式調查法,以自編式結構性問卷「中級救護技術員在職教育課程內容需求量表」為研究工具,對新北市消防局之中級救護技術員進行普查,回收有效問卷1,227份。結果:㈠新北市之中級救護技術員需求最高的在職教育課程內容為「病人評估與處置」。㈡專責救護隊及服務單位月出勤頻率≥ 300次的中級救護技術員對緊急救護基本概念、緊急救護技術、病人評估與處理、自我防護及溝通與情緒管理等課程內容的需求皆高於其他服務單位或單位月出勤頻率組別;個人週出勤頻率為「10-19次」及「20-29次」的中級救護技術員對「緊急救護基本概念」、「緊急救護技術」的課程需求皆高於「0-9次」者。結論/實務應用:建議在職教育訓練相關單位未來規劃中級救護技術員在職教育訓練時,能依不同單位屬性安排在職教育訓練課程,專責救護隊、出勤頻率高的單位增加高階緊急救護、自我防護及溝通與情緒管理之訓練課程。

Background: Emergency medical technicians (EMTs) working for the fire department provide emergency medical care to critical patients. They play a key role in the emergency medical system and conduct initial prognoses of patients in the field. The complex nature of emergency medical situations requires that EMTs participate in continuous in-service education programs to improve their emergency medical service capabilities.Purpose: This research studied current in-service education programs for Class II EMTs (EMT-2s) to identify areas / issues in need of improvement.Methods: Our research adopted a cross-sectional survey method and used a self-constructed structured questionnaire as the primary research tool. The questionnaire was provided to all registered EMT-2s employed by the New Taipei City Government Fire Department. We recovered 1,227 valid samples.Results: (1) Respondents identified education addressing ”patient assessment and treatment” as of highest priority to EMT-2 in-service education programs. (2) Respondents serving in advanced-life-support (ALS) and other units with high service frequencies (i.e., over 300 services / month) expressed stronger demand for training on basic EMS concepts and techniques, patient assessment and treatment techniques, self-protection, communication skills, and emotional management than their peers in other units. EMT-2s with 10-19 and 20-29 services per week expressed stronger demand for training on basic EMS concepts and techniques than those with 0-9 services per week.Conclusions: We suggest that when developing new or revised training modules, the training authority adjust in-service training programs to address different EMS unit characteristics. ALS and other units with high service frequencies should receive more advanced-life-support, self-protection, communication skills, and emotional management training.
