隅角閉鎖型青光眼之視神經盤壓力分析

An Analysis in Pressure Distribution on Optic Nerve Head of Angle-Closure Glaucoma

摘要


本研究以建立數值模型對隅角閉鎖型青光眼的生成與發病機制進行模擬與分析,設計出包含眼球前後房、水晶體、玻璃體、視神經盤、鞏膜、篩版的完整眼球模型,模擬從後房房水生成到受阻,以致於眼壓升高對於水晶體、玻璃體產生壓迫的狀況,做出一套完整的模擬,探討視神經盤受到壓力產生的影響。 根據本研究兩種模型條件模擬出來的結果,發現隅角閉鎖型青光眼在眼內產生的壓力具有傳遞的情形並影響眼球後方的視神經,導致視神經受到壓迫而產生視野缺陷,並且可看出眼球前後房部分,模擬出來的壓力皆符合隅角閉鎖型青光眼臨床壓力值範圍,於視神經盤部分,兩種模型條件下應力圖與應變圖趨勢相比差距不大,與文獻結果趨勢相符。並且得到傳遞至水晶體、玻璃體、篩板、視神經盤、鞏膜整體壓力分佈的狀況,此部分的壓力為臨床上還不能量測到的壓力。本研究建立出了一個完整眼球模型,可以做為往後分析隅角閉鎖型青光眼的一個依據。對往後相關的研究,提供了一個具有參考價值的數值模擬分析。

In this study, to establish numerical model for angle-closure glaucoma pathogenesis generation and simulation and analysis, design includes eye with anterior chamber, posterior chamber, lens, vitreous, optic nerve head, Sclera, lamina, the full eye model, simulation from Posterior chamber aqueous humor formation to blocked, So that intraocular pressure increased caused lens, vitreous oppressive situation, make a complete set of simulations, exploration of optic nerve head under pressure impacts. According to the study results of the two models to simulate, found that in angle closure glaucoma intraocular pressure generated with the transfer situations and affect the optic nerve behind the eye, leading to optic nerve head compression and caused vision defects, and we can see part in the eye with anterior chamber, posterior chamber, simulated pressures are in compliance with angle closure glaucoma clinical pressure range, part in the optic nerve head, two models stress figure and strain figure to the trend is compared did not differ much, trends consistent with the literature results. And get transmitted to lens, vitreous, optic nerve head, sclera, lamina, overall pressure distribution situation. This part of the clinically pressure are can't be measured. This study established that a complete eye model, can be used as future analysis of angle-closure glaucoma one basis. For future related research, provides a valuable reference for numerical simulation analysis.
