罹患口腔癌病患影響其家庭功能相關因素之探討

摘要

本研究為描述性相關研究，目的是探討口腔癌病患家庭功能運作情形，與影響其家庭功能的相關因素。採結構式問卷調查法，共訪談病患、家屬各176人。家庭功能量表採用蕭淑貞（民84）所發展的「精神病患家庭健康護理評估工具」中的「家庭功能量表」（民85再版）;社會支持量表，參考湯麗玉「癡呆症老人照顧者的功能性社會支持量表」，修訂為本研究對象適用的口腔癌病患家庭社會支持量表。資料及統計分析後，得到以下研究結果：口腔癌病患平均年齡為51.75歲;以已婚、中年男性為主，病患在家中主要角色是以父親為主。口腔癌病患的照顧家屬平均年齡為42.10歲;以已婚、女性為主;照顧家屬與病患的角色關係是以配偶為主;照顧家屬在家庭的角色是以家庭主婦為主;病患家中主要經濟來源（除病患外）是病患的子女;同住家人每月總收入在二萬元以下;病患的家庭型態是以小家庭為主;病患的家庭發展階段以中年家庭為最多。口腔癌病患的整體家庭功能得分介於中上居多（指各家庭功能比率平均值大於整體家庭功能比率平均值）。口腔癌病患家庭較需要社會支持的向度有：「有人可提供家人社會福利的訊息」、「有人可提供家人醫療相關訊息」、「有人可告訴家人照顧口腔癌病患的方法」。滿意程度最高的題項是「有人可告訴家人照顧口腔癌病患的方法」、「有人可提供家人醫療相關訊息」;「有人會給家人提醒、建議與指點」。整體來說，口腔癌病患家庭所受影響的重要因子有病患的年齡、病患的家庭型態、家庭訊息性需要程度、家中訊息性滿意程度。病患的年齡越大、病患的家庭型態發展階段層次越高、家庭訊息性需要程度越低、家中訊息性滿意程度越高，其家庭功能越好。

The objectivity of this descriptive study was to find out the family function of oral cancer patients and factors influencing them. We use structural questionnaires and interview 176 patients and families and use the ”Family function table” developed by Hsiao for health of the psychiatry family in 1995, “the modified social support table” developed by Tan for caregivers in dementia patients. The statistic analysis results are as followed, the average age of oral cancer patients are 57.75-year-old, married, middle age man, as the father in the family. The average age of the family caregiver is 42.1-year-old, married, female, spouse of the patient. The occupation of the caregiver is being a housewife. The main financial support, except the patient, in the family is patient’s son and daughter, the monthly income of the family living together is below NT$20,000; the family type of the patient is the compact one; the family development of the patient’s family is in their middle-age stage. The global family function assessment of the oral cancer patient is above the average (the perspective average family function is more than global family function). The needs in social support of the oral cancer patient are “information about social welfare” “information about medical care”, “information about caring the oral cancer patient” The most satisfied items are “information about caring the oral cancer patient.”, “information about medical care, advices, suggestions for the family”. In global, the important factors influencing the family of the oral cancer patient are patient’s age, the stage of the family development, the informative need of the family, the informative satisfaction of the family. The older the patient, the higher the development of the family, the lower the informative need, the higher informative satisfaction constitutes a good family function.
