麻醉護理人員繼續教育需求及其相關因素

The Needs of Continuing Education in Anesthetic Nurses and Its Related Factors

摘要


台灣長期以來,對麻醉護理人員的教育訓練、資格認証及工作範圍未有明確規定;故本研究旨在探討麻醉護理人員繼續教育之現況、接受繼續教育之需求以及對人員培訓與認証之期望,並分析其影響的相關因素。採橫斷式描述及相關性研究設計,以研究者自擬的結構式問卷,調查南部三縣市之醫學中心及區域醫院,共得有效問卷243份。 研究結果:(1)個案多數期望由醫學中心開班訓練,並建立標準的課程模式,和推動資格認証及分級制度;(2)個案在過去一年接受繼續教育的平均鐘點數為16.32小時;(3)90.5%及85.6%的個案因「地點太遠」及「時間不合適」而不參加該次繼續教育活動;(4)繼續教育需求之標準化得分為68.32,對各主題的需求介於很需要與需要之間,需求強度最高的主題為「心電圖判讀」,其次為「簡易X-ray判讀」、「心臟手術麻醉」;(5)年齡愈輕、麻醉工作年資愈淺、任職於醫學中心、從事臨床麻醉前具一年以上急重症病房經驗、非麻醉護理學會會員及負責麻醉相關輔助工作的個案對繼續教育的需求較高;(6)服務醫院層級、麻醉護理學會會員與進修學位能顯著地預測繼續教育需求,預測的總變異量分別為4.4%、4.1%及1.7%,合計共10.2%。基於以上發現,建議麻醉護理學會整合一套合於需求的培訓計劃,及藉由証照制度的推動來提升麻醉護理人員的專業地位,並利用有效的措施增加繼續教育的可近性,以提高人員參與的意願。

The education, qualification, and purview of anesthetic nurses were not made a definite rule in Taiwan for a long time. The purpose of this study was to describe the present conditions of continuing education, to investigate the expectation of training, qualification, and the need of continuing education, and to analyze the related factors. A cross-sectional descriptive and correlative design was applied in the medical centers and regional hospitals of three southern cities in Taiwan. 243 completed questionnaires were analyzed. The study results included: (1) Most subjects expected to have standard curriculums, qualification, and clinical ladder for anesthetic nurses and to be trained by medical centers; (2) The average time spent for the education in the past year were 16.32 hours per subject; (3) The reason of failure to attend continuing education in 90.5% of subjects were class in far distance and 85.6% were improperly class time; (4) The standard score of the need of continuing education were 68.32, the need of each topic was between “extreme” and “ordinary”. The needed courses ranked from the highest accordingly were EKG reading, basic X-ray reading, and anesthesia for cardiac surgery; (5) Those who were younger, involved in anesthesia with fewer years, worked in medical centers, chased for diploma, had more than 1 year ICU working experience before clinical anesthesia, were not members of the anesthetic nurse association, and engaged in anesthesia-associated auxiliary work had higher need for the continuing education; (6) The status of hospital, membership of anesthetic nurse association, and chasing for diploma could predict the need of continuing education and explain the predict variance respectively is 4.4%, 4.1%, and 1.7%, the total amount of variance is 10.2%. Based on the results, we suggested that anesthetic nurse association should integrate an appropriate training program, promote the professional status by qualification system; and use effective methods to increase the feasibility and participation of continuing education.
