摘要


在心臟疾病之診斷上心電圖(ECG)扮演著重要的角色,由於心電圖直接反映著心臟各部位之電位變化情形,因此藉由觀察心電圖的波形、大小等即可判讀出心臟的健康情況。然而在心電圖之量測上除了在醫院所進行的床邊監測外,針對偶發性心臟疾病進行長時間的心電圖監測業已成為必要的診斷方式。而此類長期監測紀錄裝置最常見者即為霍特式(Holter)心電圖記錄器。但這種傳統式的霍特式記錄器,隨著所記錄時間的增長所儲存之資料量也隨著增加,進而使得記憶裝置的價格與體積隨之上揚。 本研究主要目的在於建構一可攜式且具有不整脈偵測能力之心電圖記錄器,藉由不整脈事件觸發進行資料儲存的動作,如此僅針對不整脈的心電圖波形做紀錄,不但可大量減少所需儲存的資料量,尚可達到波形之先期篩選,並幫助第一線之辨識人員縮短所需之識別時間與工作量。 在系統架構規劃上,採用低功耗微處理器(MSP430)進行心電圖訊號之擷取與數位濾波,且透過R波偵測與不整脈判斷達到異常波形觸發儲存事件,而在資料儲存與紀錄方面則是使用數位式記憶裝置Compact Flash(CF)來達成,最後透過無線藍芽(Bluetooth)進行資料無線傳輸,並由接收端接收、建檔與儲存同時將所接收之病症資料以圖形化方式顯示以利後端醫護人員診視。 由以上所規劃之功能,進一步實現系統原型,且透過實驗之設計進行系統可靠度測試。最後針對所得之實測結果進行討論與分析。

Electrocardiogram (ECG) is a vital sign for the heart activities. Since the ECG traces reflect the surface potential of cardiac vector, one can monitor the heart condition by analysis the ECG waveforms. However, for occasional arrhythmia events, it is not easy to detect during outpatient clinics. A potable Holter tape system has been utilized for and long term ECG recoding for decades. The Holter system can record up to 24 hours ECG traces. These ECG data are then replyed at 60X speed in cardiac clinics for arrhythmia events detection. In general, the Holter system plays only recording function. For longer, up to a week, recording purpose, the Holter system needs large storage device and the overall dimension and cost are increasing accordingly. In this study, a lower and portable arrhythmia even recorder is developed. This device can acquire and analysis and amplify the ECG signal. Upon detected potential arrhythmia events, the device will store the ECG traces for further clinical diagnosis. Due the online screen process, the amount of the saved ECG data has dramatically reduced, so does the work load of post process by the medical staffs. The arrhythmia event recorder is consisted of an ECG amplifier, a low power consumption MSP430 microprocessor, a Compact Flash memory module, and a Bluetooth communication module. The ECG signal is acquired by the ADC of MSP430. The digitized signal is filtered and enhanced for the R wave detection and arrhythmia analysis. The detected ECG events are stored in the CF memory card and then passed to a PC via the Bluetooth module for further diagnosis purpose. The accompanied personal computer is functioned as the patient information management system. It can receive, store, display ECG wave, and also manage the patient profile and diagnostics information. The device has been tested with 48 MIT-BIT arrhythmia records. The performance showed that average sensitivity is 84.938%.
