利用拉普拉斯電位分析心房纖維性顫動之停止機制

摘要


本實驗以侵入式15×8 unipolar電極(120 channel)來量測開胸狗的心電訊號。在未施打任何藥劑前先擷取正常竇性心律(sinus rhythm)訊號,然後開始注射乙醯膽鹼(Acetylcholine,ACH)。因為注射ACH的緣故,讓心房纖維性顫動(atrial fibrillation,AF)可以持續發生下去。接著以電刺激方式來誘發AF,再分別施打Procainamide(IA)或Flecainide(IC)或d-sotalol(III)等三種不同抗心律不整藥物,來觀察AF停止之機制。 本研究主要探討心房纖維性顫動停止的機制,提出AF將停止時之最大傳導路徑較初發生心房纖維性顫動時為長之假設,以拉普拉斯電位之心內心電圖分析系統[2]來分析訊號並驗證之。共完成四筆正常竇性心律訊號及十三筆心房纖維性顫動訊號之活化等時圖,來比較正常竇性心律及AF停止前平均傳導路徑的變化,及八筆打藥前後之亂度比較。 由實驗的結果顯示傳導平均路徑在不同階段的表現及各組統計分析,其中AF的平均路徑逐漸增長,亦即:Ι(1-10)

This study uses 15*18 invasive unipolar electrodes arrays to acquire electrograms from the open-chest canines. Before injecting any drugs into canines, we took normal sinus rhythm signals first. Then, we began to inject canines with Acetylcholine (ACH) such that the atrial fibrillation (AF) could be sustained. After the atrial fibrillation was initiated using electrical stimulation, three anti-arrhythmic drugs: Procacainamide (IA), Flecainide (IC) and d-sotal (III) were injected in separate experiments to observe the mechanism of AF termination. The goal of the research is to discuss the mechanism of AF termination. We allege a hypothesis that, near termination, AF has longer mean conduction path than previous AF. Laplacian electrogram analysis system was used to analyze signals and to verify the hypothesis. Activation isochronal maps during four sinus rhythm and thirteen AF session were analyzed in order to compare the variation of mean conduction paths between sinus and AF termination. Additionally, entropies, before and after drugs injection, were also compared for 8 files,. Experimental results and statistical analyses of mean conduction path at different stages reveal that the mean conduction path before termination is progressive longer, that is AF Ι(1-10)
