剝脫性唇炎之中醫治療病例報告

病患爲十六歲的學生，主要爲上下嘴唇脫皮、皸裂已有多月，唇無粘膜紅腫、糜爛的見症，在外但治療後症狀反覆，其症狀在天氣變化、功課壓力較大、緊張會更厲害；另病患亦有身體皮膚癢，和起紅疹的情況。四診另發現病患本身喜吃冰冷物，容易疲倦，易流汗，活動不會喘，脈弦細，舌淡紅，稍有白膩苔。其症狀符合剝脫性唇炎的診斷，中醫之病名爲“唇風”或“緊唇”。本個案遵循中醫的辨證論治，認知辨爲陰陽不調，營衛不和，偏陰虛火炎較盛兼有濕阻，給予桂枝加龍骨牡蠣湯爲主方加幾味清虛熱滋陰和除濕清熱中藥共治，經過約二個月的治療後，其唇脫皮、皸裂情況已無發生，並且全身性皮膚癢和紅疹亦明顯減少，電話追蹤病患症狀穩定未再復發。此病例體現中醫的“異病同治”的想法。

The patient is a 16 years old student. The main symptom is desquamation of upper and lower lips, chap for several months. The lip has no mucous membrane with redness, rotten condition. The symptom is repeated after treatment. The symptom is worse as weather change, greater homework stress, and high tension. The patient also has skin itching, and the situation of red rash arises. After the diagnosis, it is found the patients likes to eat the ice-cold food, apt to be tired, apt to sweat, will not breathe heavily in activity, thin pulse string, light red tongue with white spot. The symptom accords with the diagnosis of the Exfoliative Cheilitis, which is called ”lip wind” or ”tight lip” in Chinese Medicine. According to Chinese Medicine syndrome, Gui Zhi Jia T ong Gu Mu Li Tang and some herbs are used to treat the patient. After treatment for about two months, the desquamation of lips, chap has been eliminated, and the skin itching and red rash has been reduced significantly. After tracing by telephone, the symptom is steady without recurring again. This case reflects the thought of ”same treatment for different diseases” in Chinese Medicine.
