滑罐對大面積反覆發作性痤瘡，結節、疤痕痤瘡的療效初探

A Medical Case Report of the Clinical Effects of Gliding Cupping on Relieving the Wide-area Atrophic Scars and Hyperpigmentation Lesions of the Patient with Acne Vulgaris

摘要

患者是32歲女性行政人員，有子宮內膜異位和車禍骨折史，2012年12月20日車禍後一日至本院傷科就診，治療期間提及背部好發痤瘡，有時發炎腫起留下凹陷性疤痕、結節或色素沉著，5年來疤痕不褪，曾至皮膚科就診仍不癒，影響美觀，故希望能淡化色素、撫平凹陷性疤痕。望診見黑白頭粉刺、色素沉著、凹陷性疤痕，膚質不油膩，無乾燥脫屑，無水泡，無紅腫化膿。問診：姿勢性頭昏，健忘，目乾澀，經後易頭痛，工作壓力大，下班疲憊喜躺臥，易脹氣，手腳易麻，肩、頸、腰、背痠痛、下肢易腫沉重，久坐臥後加重。受傷部位癒合慢、瘀青難褪。舌淡紅暗，苔薄白，有津。脈濡弱澀。證型屬氣滯血瘀，氣血虛加重血循不暢。本案例選擇使用全背部滑灌，透過病灶相鄰區域、大範圍的增加血液循環，加速代謝沉著色素，和皮膚修復，追蹤其療程圖片，發現可使淡化色素、凹陷性疤痕撫平。由此探討滑罐具潛力成為修復大面積反覆發作性痤瘡、結節、疤痕痤瘡、色素沉著的治療方法。

We report a 32-year-old female with past history of endometriosis and trauma by car accident. She mentioned that many wide-area pitting scars and hyperpigmentation lesions over her whole back since 5 years ago, while coming to our clinic for accepting the acupuncture and cupping treatment for the injury of a car accident. Observing her back, full of open and closed comedones, nodules, atrophic scars, hyperpigmentation lesions, a long-term consequence of recurrent inflamed acne lesions. She has taken a few treatments(including oral medications and ointments) during the 5years, but the efficiency is not sigificant. Therefore, we found still many comedones, nodules, atrophic scars, hyperpigmentation lesions on her back at the first examination. In addition, no oily skin, no vesicle, pale conjunctiva, postural dizziness was found, and headaches after the periods, dry eye, working stress, fatigue, less exercise, abdominal distention, limb numbness and edema, shoulder, neck and low back pain and soreness occurred, especially aggravated after bed rest. Injury and bruise healing took a long time. Purple-colored tongue with mild greasy fur, blurred, sluggish and weak pulse was observed, and all these symptoms and signs indicate the pattern of ＂Qi-stagnation and Blood-stasis worsen by Qi- Blood deficiency＂ according to Traditional Chinese Theory. We gave ＂gliding cupping＂ in her whole back as a treatment, for increasing the blood circulation over the wild area, not only on local lesions, also, enabling toxins, melanin, and metabolite trapped deep in the soft-tissue layers to be brought to the body surface. According to the pictures of the disease progress, we found gliding cupping had potential for relieving atrophic scars and hyperpigmentation.
