中藥治療膽結石絞痛之醫案報告

摘要

膽絞痛常常突然發生於有膽結石的病人，臨床上藉由超音波診斷有右上腹痛伴有膽結石通常診斷為膽石症，由於膽絞痛常在大量油膩晚餐後的夜半發生，發作6小時後症狀緩解，常造成一般民眾忽略此一疾病。本病案以中藥治療一名西醫診斷有膽石症的64歲男性，伴有中度脂肪肝、睡眠品質差。採用中藥治療以清胃熱健運脾氣，輔以下氣除滿之法，經八周治療後，膽結石縮小，脂肪肝程度降低，睡眠狀況明顯改善，而且在後續追蹤六個月內無復發膽石症病況，藉由中藥治療膽石病或許可提供不想開刀切除膽囊的病人，另一個緩解病情的好方法。

Biliary colic usually attacks Patients with gallstones suddenly. In clinical, patients with gallstones suffer from abdominal pain in the right upper quadrant usually diagnosis as gallstone disease by ultrasound. Since the biliary colic occurs in the midnight after a big meal, and the symptom will relieve in 6 hours, people ignore this disease. In this case, a 64-year-old man diagnosed as gallstone disease treated by traditional Chinese medicine. Moderate fatty liver, poor sleep quality was noted. Tradition Chinese medicine treat the patient in the method of depletion the stomach heat and supply the spleen qi , with the method of lead the qi down in order to relieve abdominal distension. After treatment in eight weeks, gallstones shrink, reducing the degree of fatty liver, sleep improved significantly, and within six months of follow-up without recurrence gallstone disease condition. Traditional Chinese medicine may be a complement option to the patients who do not want to treat biliary colic via removing the gallbladder by surgery.
