Severe Intestinal Involvement of Wegener's Granulomatosis with Concurrent Bacterial Infection in a Young Girl
一例年輕女性患有Wegener's氏肉牙腫合併嚴重的腸道侵犯及細菌感染
摘要
Wegener's氏肉牙腫是一種常侵犯呼吸道及腎臟的壞死性血管炎。腸胃道的侵犯較少見且通常是發生在超過30歲以上的病人。細菌感染可以是Wegener's氏肉牙腫復發的原因之一。我們報導一例年輕女性診斷為Wegener's氏肉牙腫同時合併有嚴重的腸胃道侵犯。此例中，克雷白氏肺炎桿菌的感染造成Wegener's氏肉牙腫的復發，而其復發的表徵即是嚴重的腸胃道侵犯。因此我們認為，及早的抗生素治療，在合併有細菌感染的Wegener's氏肉牙腫中是非常重要的。

Wegener's granulomatosis (WG) is a necrotizing vasculitis that usually affects the respiratory tract and kidneys. Clinical symptoms from gastrointestinal disease are reportedly rare and usually occur in patients over the age of 30. Bacterial infection can be a precipitant of relapse in patients with WG. Here we report a case of a young patient with gastrointestinal involvement of WG.. The gastrointestinal involvement of this patient, a sign of disease relapse, was associated with the intercurrent infection of Klebsiella pneumonia. We consider that early antibiotic therapy is important for treatment of concurrent bacterial infection in WG patients.
