流膿耳之金黃色葡萄球菌感染

Discharging Ear with Staphylococcus Aureus Infection

背景:金黃色葡萄球菌是流膿耳中常見的菌種之一,它的適應性極強,非常容易產生抗藥性。本研究目的在了解其對抗生素的抗藥性,以便使用適當的抗生素治療。 方法:我們自1995年6月至1996年5月,對門診及住院病人的流膿耳,採取外耳道膿汁作細菌培養,針對培養菌含有金黃色葡萄球菌並有完整藥物感受性記錄的40耳加以分析。 結果:發現對methicillin具感受性的金黃色葡萄球菌(methicillin-susceptible staphylococcus aureus, MSSA)菌株感染25耳,佔62.5%;對methicillin產生抗藥性的金黃色葡萄球菌(methicillin-resistant staphylococcus aureus, MRSA)菌株感染15耳,佔37.5%。MSSA對常用抗生素的抗藥性分別為:ampicillin 96%,penicillin 96%,oxacillin 0%,cefazolin 0%,gentamicin 0%,chloramphenicol 28%,clindamycin 40%,erythromycin 44%,trimethoprim-sulphamethoxazole 0%,vancomycin 0%。MRSA對常用抗生素的抗藥性分別為:ampicillin 100%,penicillin 100%,oxacillin 100%,cefazolin 100%,gentamicin 93.3%,chloramphenicol 66.6%,clindamycin 100%,erythromycin 100%,trimethoprim-sulphamethoxazole 33.3%,vancomycin 0%。

Background: Staphylococcus aureus remains an important pathogen of discharging ears. The treatment has become more difficult by the versatile human pathogen which has acquired resistance to a lot of available antibiotics. We analyzed is antibiotic resistance patterns in order to find which drug to be effective. Methods: We collected 40 discharging ears which the ear swab culture proved to contain Staphylococcus aureus from June 1995 to May 1996. We analyzed those data after they had complete drug susceptibility records. Results: The methicillin-resistant S. aureus (MRSA) was defined as the strains resistant to either methicillin or oxacillin and MSSA means the methicillin-susceptible S. aureus. Among those 40 ears, 25 ears(62.5%) were infected by MSSA and 15 ears(37.5%) were infected by MRSA. The resistance of MSSA to the commonly used antibiotics was listed as follows: ampicillin 96%, penicillin 96%, oxacillin 0%, cefazolin 0%, gentamicin 0%, chloramphenicol 28%, clindamycin 40%, erythromycin 44%, trimethoprim-sulphamethoxazole 0%, vancomycin 0%. The resistance of MRSA to the these antibiotics was as follows: ampicillin 100%, penicillin 100%, oxacillin 100%, cefazolin 100%, gentamycin 93.3%, chloramphenicol 66.6%, clindamycin 100%, erythromycin 100%, trimethoprim-sulphamethoxazole 33.3%, vancomycin 0%. Conclusions: There are very few antibiotic options in treating discharging ears with MRSA. And vancomycin was found to be the only effective antibiotics in our study.
