根管治療後前牙修復體之牙冠滲漏

Coronal Leakage of Permanent Access Restorations in Endodontically Treated Anterior Teeth

摘要

根管治療後牙齒修復體的滲漏與否關係著根管治療的成敗。隨著不同的材料與使用可能產生不同的滲漏，本研究之目的在評估前牙接受根管治療後接受光聚合樹脂(TPH)或複合體(Dyract)修復，或以光聚合樹脂(TPH)為修復體，下層墊玻璃離子體(Ketac-fil)或Caviton作為基底時，牙冠滲漏的情形。64顆前牙隨機分成每組15顆牙的四實驗組，另外4顆牙作為正、負對照組。所有牙齒完成根管治療後，、馬來膠切至牙骨質牙本質交接的高度。A、B、C、D四組的髓腔入口分別補上Dyract、TPH、Ketac-fil、和Caviton。24小時後，C、D兩組將塡補的Ketac-fil或Caviton磨除2.5mm深，外層補上TPH。兩天後，所有牙齒接受五十個冷熱循環，置於染色劑中七天，將牙齒延著長軸分開。在顯微鏡下，測量牙冠之滲漏深度及各修復物深度。結果A、B、C、D四組之平均滲漏深度為0.33mm，0.87mm，0.99mm和0.62mm，無統計上顯著差異。由此研究七天短期的觀察顯示光聚合樹脂(TPH)作為前牙根管治療後修復體時，使用Ketac-fi1或Caviton作基底，有助避免牙冠微滲漏進入基底以下的根管部分。

The purpose of this study was to evaluate the coronal microleakage of endodontically treated anterior teeth restored according to the manufacturer's instructions with Dyract (polyacid-modified composite resin), Total Performance Hybrid (TPH) (light-cured composite resin) and TPH with a base of Ketac-fil (glass ionomer), and TPH with a base of Caviton. Sixty-four intact extracted anterior teeth were cleaned, instrumented, and then filled with laterally condensed gutta-percha and sealer. The gutta-percha was removed to the cementoenamel junction. Four teeth, each two were used as the positive and negative controls. The 4 experimental groups (n=15) were restored with Dyract (group A), TPH (group B), TPH with a base of Ketac-fil (group C), and TPH with a base of Caviton (group D). After thermal cycling (5 to 55℃, 50 cycles), the teeth were coated with nail polish with the exception of the access, and immersed in basic fuchsin dye for 7 days. Microleakage was measured linearly under a stereomicroscope with an accuracy of 0.1 mm after tooth longitudinal sectioning. Wilcoxon's rank-sum test was used for the statistical analysis. The average leakages in groups A, B, C, and D were 0.33, 0.87, 0.99, and 0.62 mm. There were no statistically significant differences among the groups. However, a base of Ketac-fil or Caviton under the TPH may decrease the chance for leakage into the gutta percha.
