A 20 year-old male patient with a painless swelling mass over right mandible for half a year was presented. The incisional biopsy performed at other hospital reported a malignant sarcoma. He was later referred to our hospital. After examination and evaluation, he received operation for tumor wide excision, which resected the mandible from right parasymphysis to subcondylar region, and the mandibular defect was reconstructed with free fibula osteofasciocutanous flap. The final pathological report was undifferentiated sarcoma. Postoperatively, he received adjuvant radiotherapy for 6250 cGy. The treatment was uneventfully. The prognosis of sarcoma in head and neck region is poor. However, the patient had periodic follow up in OPD for 2 years without recurrence or metastasis. The undifferentiated sarcoma is very rare in head and neck region, so we present this case for further discussion.
