同時於雙側腮腺發生腺淋巴瘤－病例報告

腺淋巴瘤又被稱為瓦新氏腫瘤或乳頭狀淋巴性囊狀腺瘤，為腮腺內第二常見的良性腫瘤，此類腫瘤同時發生於雙側腮腺的機會約6.5%。本文報告一例70歲男性病患，每天平均抽一包菸，持續30年。因口腔鱗狀細胞癌於口腔顎面外科定期追蹤檢查，發現左側耳前方可觸及結節狀腫塊，搭配電腦斷層及細針抽吸活體檢查，疑似為腮腺腫瘤，故安排於全身麻醉下將雙側腫瘤移除，並保存面神經。目前診斷腺淋巴瘤的工具包含了組織切片、細針抽吸活體檢查、以及超音波、電腦斷層、核磁共振及核醫掃描等，隨著診斷技術進步，治療方式從以前的腮腺淺葉切除術到摘除術，甚至也有學者提出保守治療的觀點。雙側腮腺同時發生腺淋巴瘤較少見，以摘除術移除腫瘤並保留病人之面神經，可獲得良好的預後。

Adenolymphoma, also known as Warthin's tumor or papillary cystadenoma lymphomatosum, is the second most common benign neoplasm in parotid glands (accounts for 7.7% in all parotid gland tumor). The incidence of tumors occurring simultaneously in bilateral parotid glands is about 0.5-3.5%. This case is a 70 year-old male, with cigarette smoking habit (1 pack per day in average) for over 30 years, kept routine postoperative follow-up for oral squalors cell carcinoma by oral and maxillofacial surgeons. Palpable swollen nodule in bilateral pre-auricular areas were found, and were diagnosed as bilateral benign parotid tumor after computer tomography scan and fine-needle aspiration biopsy. The operation for bilateral tumor nucleation was performed under general anesthesia, and the facial nerves were preserved. The patient recovered from the procedure uneventfully, and no facial nerve injuries were noted. No local recurrence was found after follow-up period of 2 years 3 months. The diagnostic methods include tissue biopsy, fine-needle aspiration, sonography, computer tomography (CT) scan, magnetic resonance image (MRI), and Tc-99m-pertechnetate. Due to the improvement of diagnostic techniques, the treatment options from superficial parotidectomy to enucleation were advocated. Some authors propose conservative treatment, but the definite treatment protocol still remains controversial. Adenolymphomas in syuchronous bilateral parotid glands are uncommon. The prognosis is good in the cases of surgical enucleation and preservation of the facial nerve. Reviewing of the articles, the recurrence rate of the diseases is low. Various treatment protocols are taken, but long term follow-up and further investigations are mandatory.
