Contrast-Induced Encephalopathy in Patients with End-Stage Renal Disease: A Case Report and Literature Review

Patients with end-stage renal disease (ESRD) have complicated comorbidities and increased requirements for interventions and enhanced imaging. Currently, the use of a contrast agent is relatively safe, but it still causes several complications such as anaphylactic reactions, nephropathy, or encephalopathy. Here, we report the case of a patient who developed contrast-induced encephalopathy (CIE) with the presentation of sudden onset of conscious disturbance after undergoing computed tomography angiography and percutaneous transluminal angiography (PTA). His consciousness completely recovered within 2 days after 2 hemodialysis (HD) sessions. CIE is a rare and unexpected complication that might be associated with hyperosmolarity and chemotoxicity of contrast agents, causing damage of blood-brain barrier and neuronal excitability. The effect of CIE appears to be transient and reversible in dialysis-dependent patients. Physicians should be cautious regarding the conscious status of dialysis-dependent patients after contrast exposure in imaging studies and provide appropriate management.
