We report a 50-year-old female who had metastatic adenocarcinoma of the breast which mimicked a primary breast cancer with regional lymph nodes metastases. The clinical presentation and histologic findings were similar to primary invasive ductal carcinoma of the breast. We hypothesize that the origin was adenocarcinoma of the rectum which metastasized to the breast and then seeded in the ipsilateral axillary nodes.
