優質化老人用藥及藥事照護服務

老人用藥相關問題及其內外在影響因素非常複雜，致使生活周圍持續有老人用藥風險案例的發生或相關聳動的媒體報導。本文以敘述性回顧方式檢視臺灣老人用藥問題、藥事照護現況，並探討國內外相關文獻及研究結果。近來，不少臺灣藥師已漸漸從醫院藥局裡走出，進入社區及長照機構；但除了藥學教育及畢業後持續教育還需要加強並熟稔老人用藥及藥事照護的內容外，藥師們對於具備某些特質的老人要多加留意其使用某些藥物的傾向、改變及可能的後續風險，尤其是有使用潛在不適當用藥者。至今，臺灣各藥事執業機構的藥師在執行老人藥事照護服務時仍然面臨許多困境與障礙。我們可以參考美國臨床藥學的發展與正面的成果，考量本土社會、人文或醫療系統的差異，建立適合臺灣不同執業環境的老人藥事照護模式。藉此可以讓更多臺灣藥師在未來提供老人個別化的藥療管理時，能與其他醫療專業人員合作來共同解決老人用藥問題，最後終將使得我們身邊的老人因為有臺灣藥師的專業照護而多了一層用藥有效性、經濟性暨安全性保障的機會。

Drug-related problems (DRPs) and their associated internal and external factors are very complex and frequently result in the occurrence of adverse events or sensational media reports. This article employed narrative review to examine DRPs and pharmaceutical use for the elderly in Taiwan, and explored the relevant literature and research findings. Recently, pharmacists have gradually become more involved in community practice and long-term care facilities, as opposed to hospital pharmacies. Pharmacists are supposed to be knowledgeable on the aspects of medication use and pharmaceutical care in the elderly. Such topics and contents should be included in pharmacy educations and postgraduate trainings. It is important for pharmacists to pay more attention to patients within certain populations and monitor use of certain medications, drug therapy changes, and possible subsequent risks. However, there remain many difficulties and obstacles in practice sites for pharmacists to implement pharmaceutical care in the elderly in Taiwan. It is possibly to work toward establishing the appropriate pharmaceutical care models for the elderly in various practicing sites in Taiwan by referring to positive outcomes of clinical pharmacy from the United States. However, it is important to consider the differences in local social, cultural or medical systems in Taiwan. Pharmacists in Taiwan can develop the ability to provide individualized medication therapy management in the elderly and collaborate with other health care professionals to identify and solve DRPs. Advancing pharmacists' professional involvement in Taiwan will ensure effective and safe medication use for the elderly, while also being economically beneficial.
