一位慢性阻塞性肺病病人行肝臟移植手術後脫離呼吸器之護理經驗

摘要

本文描述一位慢性阻塞性肺病病人併肝硬化接受肝臟移植手術，術後脫離呼吸器之加護經驗，筆者於2015年11月10日至11月25日照護病人，運用直接觀察、身體評估、病歷查閱與病人筆談等方式收集資料，進行身、心、社會、靈性評估，發現病人有活動無耐力、呼吸機戒斷反應功能失常、焦慮等三項主要護理問題。筆者透過文獻查閱設計並執行病人肺部復健計畫，運用上肢平舉擴胸運動、下肢踏步運動搭配輔具讓病人踏出移植後的第一步。呼吸訓練過程中，病人生理及心理均受到及大的壓力，需要護理人員，傾聽其內心感受，給予正向支持，激發病人脫離呼吸器的決心，協助病人順利脫離呼吸器，藉分享此照護經驗，提供護理人員臨床照護病人之參考。

This article describes the nursing experience from caring a patient with COPD and liver cirrhosis accepting liver transplant surgery weaning ventilator. The author applied observation, body assessment, medical records review, written conversation with patient to assess the physiological, psychological, social and spiritual conditions of the patient from November 11st to 25^(th) in 2015. The result showed three major nursing issues, namely, dysfunctional ventilator weaning responses, activity intolerance and anxiety. Through literature review the author designed a series of pulmonary rehabilitation plans such as upper extremity chest lift exercise and lower limb stepping exercise. This plan encouraged patient to take the first step after the transplant surgery with assistive devices. The patient was both physiologically and psychologically stressed during the breathing training. The patient requires the nurse listening to him, provides positive support, and encourage his determination to break away from weaning ventilator. We hope this case report provides the medical teams some references for further clinical care.
