動態肌筋膜放鬆術對慢性頸部疼痛的立即療效

Immediate Effects of Dynamic Myofascial Release for Chronic Neck Pain

摘要


背景與目的:頸部疼痛是一個具高盛行率的肌肉骨骼疾患,其中有35%以上的患者會演變成慢性頸部疼痛。本研究將患者主動關節動作結合肌筋膜放鬆術的治療方式稱為動態肌筋膜放鬆術,希望以此方式改善慢性頸部疼痛患者的肌筋膜受限、疼痛與功能表現,並比較與牽拉運動的差異。研究方法:共收取34位慢性頸部疼痛個案,隨機分配為動態肌筋膜放鬆術組與牽拉運動組,兩組均進行約15分鐘的介入,在介入前後分別進行療效測量,包括自覺疼痛症狀、肌肉壓痛閾值、頸部關節活動度、頸部失能狀況與恐懼迴避信念。結果:動態肌筋膜放鬆術組在自覺疼痛症狀嚴重程度、頸部關節活動度、頸部失能狀況、恐懼迴避信念上在治療後皆有顯著改善,肌肉壓痛閾值則無顯著差異。而牽拉運動組則在自覺疼痛症狀嚴重程度、頸部失能狀況上也有顯著改善,其它則無顯著差異。兩組療效的比較,在自覺疼痛症狀嚴重程度、頸部關節活動度、頸部失能狀況上,動態肌筋膜放鬆術的改變量顯著大於牽拉運動,其它則無顯著差異。結論:根據本研究結果可證實動態肌筋膜放鬆術可立即改善慢性頸痛個案的自覺疼痛程度、頸部關節活動度、頸部失能狀況與恐懼迴避信念,且效果顯著優於牽拉運動。

Background and purpose: Neck pain is high prevalence of musculoskeletal disorders, which more than 35% of patients develop into chronic neck pain. The study have therapy of active movement with myofascial release by patient that is called dynamic myofascial release. Hoping to improve the way patients with chronic neck pain is limited myofascial, pain and functional performance, and compared the difference between stretching exercise. Methods: Total received 34 subjects with chronic neck pain. Randomize assigned to a dynamic myofascial release and stretching exercise group. Both groups were intervention in about 15 minutes. Before and after the intervention were measured that involve conscious pain, muscle pressure pain threshold, range of motion, neck disability and fear avoid belief. Results: Dynamic myofascial release group is significant improvement after treatment on conscious pain, range of motion, neck disability and fear avoid belief, but it's not significant improvement on muscle pressure pain threshold. Stretching exercise group is significant improvement after treatment on conscious pain and neck disability, but others is not significant improvement. In compared between groups, dynamic myofascial release was better than stretching exercise on conscious pain, range of motion, neck disability, but others is not significant improvement. Conclusion: Results of the study demonstrate immediate improve of dynamic myofascial release for conscious pain, range of motion, neck disability and fear avoid belief of chronic neck pain, and better than stretching exercise in results.
