以心率變異度及量表探討頸椎牽引過程中角度對療效之影響

The Effect of Angle in the Process of Cervical Traction by Measuring Heart Rate Variability and Scale

摘要


背景與目的:頸椎退化性關節炎或椎間盤突出常引發肩頸痠痛,這是常見的疾病之一。近年來,消費性電子產品與日俱增,因不當的使用習慣而造成肩頸疼痛的患者比以往增加,而且年齡層逐漸下降。頸椎病的治療大多採用藥物與復健的保守療法,而頸椎牽引是復健療法中最常見之一。坐式牽引的療效主要受到牽引力量以及牽引角度的影響,其中牽引角度較易在牽引過程中發生角度偏移,本研究將進行其對療效影響之探討。方法:利用三軸加速規監測牽引過程中角度的偏移狀況,並以視覺疼痛類比量表、頸椎功能障礙指數量表、心率變異度分析和頸椎活動度量測儀針對實驗組與對照組進行牽引前後之療效評估。結果:兩組的視覺疼痛類比量表都有明顯差異,但對照組的疼痛緩解程度較大。在頸椎功能障礙指數與心率變異度分析上,對照組有較明顯差異。結論:牽引對疼痛緩解具有明顯療效,但若於牽引過程中發生角度偏移,將會降低牽引之療效。

Background and Purpose: Cervicobrachial pain is a common disease that is often caused by herniated intervertebral disc or cervical spondylosis (arthritis of the neck). In recent years, the improper use of electronic products caused more young patients to suffer from neck pain. Treatment for neck pain generally starts with a conservation approach such as medication and physical therapy, however, cervical traction is a common physical therapy. The effectiveness of cervical traction is mainly affected by the traction forces and angles. Since traction angles are easier to shift, the aim of this study was to investigate the influence of angle excursion during cervical traction treatment. Methods: Three-axis accelerometer was used to detect the traction angle. And Visual Analog Scale (VAS), Neck Disability Index (NDI), heart rate variability (HRV) and cervical range of motion (CROM) were used to assess the therapeutic responses in both the treatment and control group before and after the cervical traction. Results: Both groups' VAS scores were reduced after treatment, and there are significant differences. Nevertheless, pain relief is better in control group. In addition, the significant differences in NDI and HRV were just occurred in control group. Conclusions: Our findings show not only that cervical traction treatment is obviously beneficial for patients with neck pain, but also that angle excursion could decrease the curative effect.
