體感遊戲對於巴金森氏病患者平衡之療效:統合分析與系統性回顧

Effects of Exergame on Balance in Individuals With Parkinson's Disease: A Systematic Review With Meta-Analysis

摘要


背景和目的:巴金森氏病對於患者動作功能的影響包括姿勢不穩、平衡受損、步態異常等,而這些異常的表現將會影響他們日常生活的功能性活動,增加跌倒的風險。體感遊戲結合了身體適能和認知層面的訓練,藉由視覺、聽覺、其他感覺的回饋,進而提升運動表現。本研究目的為利用統合分析與系統性回顧,探討體感遊戲對於巴金森氏病患者在平衡方面的改善成效。方法:本文搜尋相關電子資料庫,納入過去至2018年4月前的隨機對照試驗。使用皮卓量表與新版牛津實證醫學中心證據等級分析文獻的品質與證據等級。若有2篇以上,使用相同平衡測試方法,則使用Review Manager 5.3軟體(The Cochrane Collaboration, Copenhagen, Denmark)進行統合分析。結果:本研究納入6篇文獻進行統合分析和文獻回顧,統合分析結果顯示巴金森氏病患者在經過體感遊戲介入後,於伯格氏平衡量表上有顯著的療效(mean difference [MD] = 1.94, 95% confidence interval [CI]: 0.87~3.00, p = 0.0004)。而根據系統性文獻回顧可知,體感遊戲也有降低跌倒風險與改善生活品質的效果。結論:體感遊戲對於巴金森氏病患者的平衡表現有顯著性的改善效果。

Background and purposes: People with Parkinson's disease (PD) often experienced movement impairment such as postural instability, balance impairment, and gait dysfunction. These impairments would cause difficulty in managing functional task in activities of daily living and would increase risk of falling. Exergame training involves giving out tasks in virtual environment to enhance physical and cognitive layers of human bodies. During performing tasks in virtual environment, augmented visual, auditory, and sensory feedback are provided to participants so that they can measure and set new goal to improve performance. The purpose of this study is to systematically review the evidence of exergame in randomized control trial and used a meta-analytical approach to assess the effects on balance in people with PD. Methods: A systematic search of the randomized controlled trial studies to the end of 31st March 2018 was conducted. Some related databases were searched. The quality of eligible studies was evaluated using PEDro Scale and Oxford classification. Meta-analysis was conducted using Review Manager Version 5.3 software (The Cochrane Collaboration, Copenhagen, Denmark). Results: Eventually, 6 studies were included in meta-analysis and systematic review. The results of meta-analysis showed that exergame significantly improved Berg Balance Scale in people with PD (mean difference [MD] = 1.94, 95% confidence interval [CI]: 0.87 ~ 3.00, p = 0.0004). According to systematic review, exergame also decreased fall risk and improved the quality of life. Conclusions: Exergame may help to improve balance performance in people with PD.
