不同身體活動量之社區老人骨質密度、骨質代謝指標與功能性體適能的差異

Comparison of Bone Mineral Density, Bone Turnover Markers and Functional Fitness Among Community-Dwelling Elderly with Different Physical Activity Levels

摘要


本研究目的為探討不同身體活動量之社區老人其骨質密度、骨質代謝指標、飲食鈣攝取量及其功能性體適能的差異情形。研究中共有182位社區老人完成試驗,使用臺灣老人身體活動量表(Physical Activity Scale for the Elderly, PASE)做為身體活動量分級之依據分為高活動量組、中度活動量組以及低活動量組,每位受試者需進行骨密度檢測、血液骨質代謝指標、飲食調查分析以及功能性體適能測驗。研究結果為:骨密度T值(T-score) 在高活動量者為-1.8 ± 1.5、中活動量者為-1.9 ± 1.5、低活動量者為-2.4 ± 1.4,三組比較並無顯著差異;血液骨質代謝指標:Beta-骨膠原蛋白碳末端肽鏈、骨鈣素,以及飲食鈣攝取量三組間比較亦無顯著性差異。功能性體適能結果顯示:高身體活動量者,其上下肢肌力、敏捷性及心肺耐力等能力顯著優於低活動量者。本研究結論是不同活動量社區老人其骨質密度T值與骨質代謝指標差異不顯著,但是維持高日常身體活動量者在功能性體適能上則呈現較佳狀況。

The aim of this study was to compare the differences in bone mineral density, bone turnover markers, dietary calcium intake, and functional fitness among community-dwelling elderly individuals with different levels of physical fitness. One hundred and eighty-two elderly subjects completed the Physical Activity Scale for the Elderly (PASE), and were divided into three different levels of physical fitness groups, including a high level group (high levels of physical activity, PA-H), a moderate level group (moderate levels of physical activity, PA-M) and a low level group (low levels of physical activity, PA-L). Bone mineral density, bone metabolism biomarkers, dietary records, and Functional Fitness Tests were assessed for all participants. In the results, bone density test showed no significant difference in T-scores among the three groups (PA-H: -1.8 ± 1.5; PA-M: -1.9 ± 1.5; PA-L: -2.4 ± 1.4). We further noticed that bone metabolism makers (β-CrossLaps [β-CTx] and osteocalcin) and dietary calcium intake were not significantly different among the groups. However, the upper limb muscle strength, agility and cardiopulmonary endurance were significantly greater in the PA-H group than those in the PA-L group. This study concludes that bone mineral density and bone turnover markers were no significant differences in elderly populations. However, maintenance of higher physical activity levels in elderly population is associated with better functional physical fitness outcomes.
