Nursing Experience of an Adolescent with Ovarian Cancer Receiving Initial Outpatient Chemotherapy

摘要


本文主要描述一位初次診斷卵巢癌青少女接受化學治療之門診照護經驗。筆者於2016年8月22日至2016年12月30日四個月護理照護期間,經8次門診訪視及4次電話訪談,藉由整體性的健康評估以會談、觀察、電話訪談、身體評估等收集病人生理、心理、社會及靈性之健康問題,加以分析資料,確立個案主要健康問題為:疼痛、焦慮及身體心像紊亂。筆者身為門診護理師,觀察到近年來門診病人,罹癌的年齡層有年輕化的趨勢,於初次診斷即時介入,藉由關懷、陪伴、支持與信任關係的建立,可增加個案面對疾病療程的信心,緩解癌症治療過程中的不適及有效減低疾病治療壓力。提供完整的疾病資訊與護理指導,安排成功的個案經驗分享,期許能幫助個案慢慢接納外觀的改變,重新燃起對未來的希望而完成治療,以適應治療後的新生活。過程中仍以回診、電訪、持續性照護,促使癌症病人獲得更完善的照護與協助。

This report mainly describes the nursing experience in caring for an adolescent who was initially diagnosed with ovarian cancer and is receiving initial outpatient chemotherapy. The patient was cared for over a 4-month period from August 22 to August 30, 2016. After 8 follow-up visits and 4 telephone interviews, the author employed holistic assessment to collect and analyze data on physiological, psychological, social, and spiritual health problems from observations, interviews, and telephone interviews. The main health issues identified included pain, anxiety, and body image disturbance. As an outpatient nurse, the author has observed that the age of outpatient patients with cancer shows a decreasing trend; patients are becoming younger and younger. Timely intervention after initial diagnosis and establishing a relationship of care, companionship, support, and trust can increase the confidence of the patient regarding the course of treatment, alleviate discomfort during this process, and effectively decrease treatment stress. Complete disease information and care guidance were provided and experience-sharing was arranged with successful patients to help the patient slowly accept changes in her appearance and reignite hope in her to complete treatment and adapt to a new life after treatment. During this process, consultation, telephone interviews, and continuous care were carried out to enable the patient to obtain more complete care and assistance.
