新式美觀乳牙冠－紀氏乳牙白冠（CWCs）的臨床應用

摘要

背景：多年來，不鏽鋼乳牙套被認為是最耐用、經濟且可信賴的復形方式。然而，牙齒顏色的復形材料比較容易受到孩童和家長的青睞。因此，考慮乳牙復形方式的因素不應只是功能及耐用性，也應考量美觀因素。目的：本文介紹紀氏乳牙白冠（CWCs）的製作方式並描述三例以CWCs復形嚴重缺損乳臼齒的病例。術後每三個月的回診，醫師會評估牙套的臨床結果並以問卷調查家長滿意度。病例報告：本文病例一在鎮靜麻醉下進行治療，病例二及病例三則在門診完成治療。在一年以上的追蹤期間，所有CWCs周圍牙齦健康、沒有染色、沒有破裂並有適當的咬合接觸。家長滿意度高（平均9.5/10分）且對外觀最為滿意，並以CWCs作為未來復形的首選。討論與結論：紀氏乳牙白冠為乳臼齒的美觀牙冠，以PMMA製成。其優點有牙齒色的外觀、較少的牙齒切削量、極少的對咬牙磨耗、術後較易維持牙周健康及牙齒貼合度佳等。術後追蹤家長滿意高並有良好的穩定性。對於大範圍缺損的乳臼齒，CWCs可作為美觀復形的另一種選擇。

Background: For years, stainless steel crowns (SSCs) are considered to be the most durable, economical and reliable restorations for grossly damaged primary molars. However, both children and their parents prefer tooth-colored restorations. Thus, the choice of full coverage restorations for primary teeth must provide not only restoring function and durability, but an esthetic appearance. Aim: This article presents the clinical application of CWCs and three cases where grossly damaged primary molars were restored with CWCs. At recall, clinical outcomes were assessed by dentists and a questionnaire was used to evaluate parental satisfaction with CWCs Case reports: Case 1 received dental treatment under IV sedation. Case 2&3 were treated after behavior management. All CWCs were free of gingival inflammation and did not exhibit color change at least 1 year after treatment. All crowns were retained without fracture. The score of parental satisfaction was high with a mean score of 9.5 out of 10. Parents were most satisfied with the crown's appearance. They would still choose CWCs for future treatment. Discussion and conclusion: Chi-white crowns (CWCs) are esthetic crowns for primary molars. They are made entirely by polymethyl methacrylate (PMMA). The margin of the crown could be adjusted and relined. These features not only provide better margin and internal adaptability of crown to tooth, but minimize tooth preparation. CWCs showed clinical success after one year and clearly met the esthetic expectations of parents and patients. In conclusion, CWCs is a new approach and another esthetic alternative for damaged primary molars.
