Background and Overview: Inflammatory dentigerous cysts are frequently found in young patients. They develop as a result of the intra-follicular spread of periapical inflammation from the overlying primary tooth. Case Description: The authors report a rare case of consecutive development of three inflammatory dentigerous cysts associated with impacted mandibular premolars and a maxillary second premolar. Through marsupialization of the cyst, followed by orthodontic traction of the impacted maxillary premolar, the underlying three permanent premolars were successfully brought into their proper positions. Conclusions and Practical Implications: In a large dentigerous cyst with displacement of permanent teeth, marsupialization is the first surgical choice to provide favorable outcomes and permit eruption of the permanent teeth.
