以ISBAR提升兒科單位內護理交班完整率之專案

A Project Using ISBAR to Improve the Nursing Shift Handover in the Pediatric Ward

摘要

交班為護理人員延續照護的工作內容之一，訊息傳遞不正確，會造成醫療異常事件發生。醫策會醫院醫療品質及病人安全政策，更強調醫療照護人員間溝通的有效性。本單位稽核小組於2011年6月1日至6月7日發現單位內交班不完整率達41.1%，不完整的原因有：沒有統一的交班標準、缺乏符合單位特性的交班單、未建立交班稽核制度。設定目標為交班完整率85%，進行改善措施包括以ISBAR原則修訂單位內交班作業規範標準，宣導及實施單位內交班作業規範標準，訂定交班單張，擬定及執行教育訓練辦法，並舉辦在職教育，制定稽核表及稽核制度。結果：本專案實施後交班完整率由58.9%提升至90.1%，交班滿意度由33.3%提升到100%，不僅使單位內交班完整率提升，交班訊息完整，病人住院更加安全，且更能提升護理品質。

A detailed nursing shift handover is important for nursing staff to continue with the work of patient care. Any mistake or misunderstanding during the nursing shift handover could lead to medical adverse events. The policy of Joint Commission of Taiwan regarding the patient care and quality emphasizes the efficacy of the communication between staff members. From June 1 to 7, 2011, we found that the rate of incomplete nursing shift handover was 41.1%. Inconsistent standard of communication, the lack of structured sheets, and the lack of an established audit system may contribute to the low rate of complete nursing shift handover. To improve the problem, we used ISBAR to revise the Nursing Shift Handover Guideline and set a goal of completion rate of 85%. We established the standard of communication sheets, the continuous education and training for the new staff, and the implementation of the audit system. Based on this project, the completion rate of nursing shift handover has been increased from 58.9% to 90.1%. The rate of satisfaction in the nursing shift handover was also improved from 33.3% to 100.0%. This project has positive impact on the nursing shift handover, and thus improved the quality of care and patient safety in the pediatric ward.
