Greater trochanter tip fracture is a predisposing factor of iatrogenic femoral neck fracture during nailing for shaft fracture - A case report

Antegrade femoral nailing has emerged as the standard treatment for femoral shaft fractures. Although a high union rate and few complications have been reported for this method, an iatrogenic femoral neck fracture is an uncommon complication. We report the case of an iatrogenic femoral neck fracture that occurred during nailing for a concomitant femoral shaft and greater trochanter tip fracture. The fracture line extended from the edge of the greater trochanter fracture to the basilar neck. Subsequently, further violation of the anterior cortex of the trochanter was noted during revision surgery. After bed rest for 4 weeks and non-weight-bearing mobilization for an additional 8 weeks, both the fractures involving the femoral neck and shaft united. An acceptable functional outcome was obtained eventually.
