Cervical infectious spondylitis mimicking cervical spine fracture-dislocation: A case report

Herein, we describe a case of cervical infectious spondylitis mimicking cervical spine fracture-dislocation. A 59-year-old man presented with severe neck pain radiating to the right upper limb. The initial cervical spine radiograph showed findings of cervical spine fracture-dislocation: C6 fracture with vertebral body destruction, C6-7 retrolisthesis, and associated widening of the interspinous space. However, cervical spine infection was diagnosed based on the results of laboratory tests and magnetic resonance imaging (MRI): elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) concentration and C6-7 infectious spondylitis with paraspinal and epidural abscesses at C5-T1. The patient completely recovered after surgical intervention and antibiotic treatment without uneventful neurological sequelae. Thus, cervical spine infection may mimic cervical spine fracture-dislocation on initial radiographic examination. Therefore, obtaining a detailed patient history, combined with advanced imaging and laboratory studies, can help conduct a differential diagnosis and select the appropriate corrective treatment.
