Pediatric physeal ankle fractures with intra-articular involvement require precise anatomical reduction to heal correctly. Although open reduction and internal fixationyielded promising results, iatrogenic injuries and postoperative arthritis remain concerns. Herein, we report two cases of Tillaux and triplane fractures treated using arthroscopically assisted reduction and internal fixation. This surgical technique is quite common for the treatment of ankle injuries in adults, but it is seldom reported in the pediatric field. In our cases, the outcomes were optimal, both patients resumed full activity within 2 months, and the American Orthopedic Foot and Ankle Society scores were 100 at the 1-year follow-up. With arthroscopic assistance, a smaller surgical wound and precise reduction are possible even in pediatric patients.
