摘要

臨床上當病人有血氧不足、缺氧或呼吸衰竭情況時，常以氧氣面罩給氧做為處置，然而長期配戴氧氣面罩，常因此導致鼻樑、臉頰、耳廓處壓紅或壓傷。預防的方式就是以人工皮局部覆貼鼻樑和臉頰，及以紗布包裹固定帶以保護耳朵，但人工皮價格昂貴，又須經由護理人員個別剪裁後黏貼，紗布粗糙又無緩衝效果，長期下來既影響患者的照護品質，對家屬的經濟或是護理人員的工作亦形成負擔，故激發本設計動機。本設計之防壓傷氧氣面罩，有以下四個特點：（1）呼吸面罩內與臉頰接觸部位增設矽膠襯墊，既貼合臉部又有減壓功能。襯墊與面罩一體成形，不須另外剪裁；（2）加寬固定帶，減輕臉頰皮膚壓力；（3）掛耳式固定帶，可去除因躁動導致面罩鬆脫的情形；（4）掛耳式固定帶上增加活動式矽膠管，可配合使用者臉型調整位置；（5）面罩上的鋸齒孔洞設計，利於固定鼻胃導管，避免皮膚受壓。期望未來能與廠商合作量產以降低成本，並推廣應用於臨床實務中，增進患者的照護品質、減省護理人員不必要的工時及降低照顧者的經濟負擔。

Oxygen masks are commonly used to deliver oxygen to patients who have hypoxemia, hypoxia, or respiratory failure. However, the long-term use of oxygen masks often leads to redness or bruises on the nasal bridge, cheeks, and auricles. This can usually be prevented by covering the nasal bridge and cheeks with artificial skin and wrapping gauze around the ears. However, artificial skin is expensive and must be prepared for individual patients by nurses, whereas gauze is rough and offers no cushioning effect. In the long term, this not only affects the quality of care received by patients but also increases the economic burden on the patient's family and the workload of nurses. These factors motivated us to design an innovative oxygen mask. Our novel anti-pressure injury oxygen mask has the following four features: First, silicone pads were added to the contact area between the oxygen mask and the cheeks. This serves to increase ventilation and reduce pressure. With the cushion built into the mask no need exists to shape cushioning material separately. Next, the elasticated headband was widened to reduce pressure on the cheeks. In addition, an ear-hook style elastic headband prevents the mask from coming loose if the patient becomes agitated. Fourth, flexible silicone tubes were added to the ear-hook elastic headband to allow the mask to be adjusted to fit the contours of the user's face. Lastly, the serrated-hole design of the mask facilitates the fixation of a nasogastric tube, if needed, and reduces pressure on the skin. In the future, we hope to collaborate with manufacturers to mass-produce this oxygen mask so as to reduce costs and promote its widespread application in clinical practice. This will help to improve the quality of patient care, decrease the workload for nurses, and reduce the financial burden on caregivers.
