On March 11, 2016, an AS365 N3 helicopter under National Airborne Service Corps (NASC), carried five members on board, including the pilot, copilot, crew chief, and two members of the Coast Guard. When preparing to hoist personnel near the cargo ship, the helicopter crashed into the sea and destroyed, two persons died and three persons were severely injured. Inspections include the following items: disassembling and cleaning the tail rotor gear box, inspecting macroscopic features of the control shaft and control rod, disassembling the control rod bearing, inspecting macro- and microscopic features of the bearing assembly, and further material tests. Finally, we find the root cause of the tail rotor gear box failure.
