以人際關係教學改善醫院護理人員之溝通自信心

Using Interpersonal Situation Teaching to Improve the Communication Self-confidence of Nurses in Hospital

摘要


背景:畢業後訓練護理人員(Nurse of post-graduated year, NPGY) 在面臨人際緊張情境互動時,可能因溝通經驗不足或溝通技巧的使用不當導致溝通成效不彰的挫敗感,甚或貶低自我價值並對自我專業成就質疑。目的:本研究探討人際情境演練介入後,NPGY 自評溝通自信心(Communication Self-confidence, CSC)提升情形。方法:採用實驗性研究法,以某醫學中心為取樣地點,參與研究之NPGY 隨機分派至控制組與實驗組,兩組在接受人際情境教學課程後,實驗組需再參與人際情境演練介入。比較兩組在人際情境教學課程前及人際情境演練介入後第一與第三個月NPGY 的CSC 自評結果成效。結果:共收案77 位,控制組42 位,實驗組35 位,平均年齡為22.9±1.2 歲,兩組間的基本人口學變項均未達統計顯著差異。實驗組前測CSC 平均分數低於控制組,第一個月後測平均分數兩組都有增加,第三個月後測實驗組繼續上升而控制組降低。結論/ 實務應用:研究結果顯示兩組NPGY 在參加人際情境課程教學後,CSC 平均分數皆有提升,實驗組在接受情境演練介入後CSC 平均分數持續上升而控制組反倒下降,雖未達統計顯著差異但呈現演練介入後延宕記憶延長之成效。建議溝通課程能結合人際情境演練模式,讓NPGY 從實境互動中提升個人溝通自信心。

Background: Nurses of post-graduated year (NPGYs) are often frustrated in a tense clinical situation, due to the unsophisticated and ineffective communication. It can further lower the self-esteem and diminish professional accomplishment. Purpose: This study investigated the increase of communication self-confidence (CSC) of NPGYs after interpersonal situation training (IST). Methods: 84 NPGYs in a medical center were enrolled in this experimental study, and were randomly assigned to the experimental and control groups. Both groups received interpersonal situation classroom teaching (ISCT), the experimental group further received the IST. The CSC self-evaluation results from two groups were compared at baseline (before ISCT), the first and third months' time after IST. Results: 77 NPGYs were included in this study with control group of 42 and experimental group of 35 nurses. The mean age was 22.9±1.2 years. There were no significant differences in all baseline demographic variables. The average scores of CSC in experimental group were lower than control group, the post test scores of the first month were increased in both groups. At the third month's follow-up, CSC scores of the control group decreased slightly while those of the experimental group continued to rise. Conclusions/Implications for Practice: The study revealed that the CSC average scores were increased in both groups after ISCT. In the experimental group, CSC scores continued to rise after IST, while CSC scores of the control group were decreased. Unfortunately, the result did not show a significant difference in improving self-confidence of nurses upon communication. However, the effect of delayed memory was shown. NPGY communication training can combine with situational interaction to enhance CSC.
