Characterising Physiological Conus Uptake in F-18 Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography

Aim: The purpose of this study was to evaluate and reproduce frequency, intensity, and associations of physiological distal spinal cord (conus) F-18 FDG uptake visible on modern time-of-flight PET/CT scanners. Method: We retrospectively reviewed 660 consecutive patients (426 males, 234 females; mean age 61; range 15~92) on a Siemens Biograph mCT 128 slice PET/CT scanner. The median scan occurred at 60 minutes post-intravenous injection of F-18 FDG (mean activity 338.85 MBq, SD=58.46). Scan indication, age, gender, weight, height, diabetes and renal function status, fasting blood glucose level (BGL), insulin use, metformin use and hydration status were collected from data recorded at the time of study. SUVmax and SUVmean were measured at defined positions and included: (1) cervical cord; (2) distal cord; (3) mediastinal blood pool (descending thoracic aorta); and (4) liver (centre of right lobe). Results: Of the 660 patients reviewed, none had conus pathology found in medical records up to two years following PET/CT. The maximal SUVs in the conus fitted well in a normal distribution. F-18 FDG uptake (both SUVmax and SUVmean) was consistently higher in the conus than in the cervical cord, mediastinal blood pool, and liver (cervical cord SUVmax β=0.477, t (659)=9.04, P < 0.001; SUVmean β=0.463, t (659)=8.82, P < 0.001; mediastinum SUVmax β=0.477, t (659)=9.04, P < 0.001; SUVmean β=0.463, t (659)=8.82, P < 0.001; liver SUVmax β=0.512, t (659)=6.84, P < 0.001; SUVmean β=0.469, t (659)=7.34, P < 0.001). Statistically significant associations were found between distal cord uptake and age, BMI, weight, fasting BGL, administered F-18 FDG activity, and scan delay. Conclusion: Distal spinal cord F-18 FDG uptake visible on time-of-flight PET/CT scanners is a physiological phenomenon which is described in detail in this study.
