協助一位思覺失調症新移民女性跨文化適應之護理經驗

本文探討一位新移民思覺失調症個案，因跨文化適應失調及無效性健康維護能力，導致幻聽、暴力等症狀反覆發作而重複入院之護理經驗，護理期間為2016年7月9日至2016年8月22日，運用觀察及會談技巧，藉由五大層面整體性評估發現其主要護理問題有：高危險性暴力、無效性健康維護能力及跨文化適應失調。護理人員引導家庭成員接納其文化價值與信念，參與照護計畫，促使個案學習調適及修正其負向情感反應，引導家屬成員間正面互動模式，改善因社會文化差異導致的矛盾家庭關係。期望藉此經驗給予照護類似個案的護理人員，做為促進個案回歸家庭與社區的參考，提升臨床上個別照護的品質。

The purpose of this paper is to report on the nursing intervention treatment to help a new immigrant schizophrenic case who suffered from auditory hallucination and violence behavior. During the nursing process from July 9th to August 22th of 2016, the authors gathered subjective and objective data through conversation and observation. Through the holistic five dimensions nursing assessment, we found that the main nursing problems in this case were: (1) high risk violence behavior (2) ineffective health maintenance (3) cross-cultural adaptation disorder. The authors explained the difference between Taiwanese and Vietnamese cultures to the case and her family members, and enable interpersonal interaction. Therefore, she could return family and community and find a part time job raising her quality of life Team integration treatment was observed to be an effective intervention in the care of this case suffering from new Immigrant feminine suffer from schizophrenia.
