背景: 台灣東部地區特殊的人文地理環境及交通不便、就醫不易等因素,造成牙科醫療資源的長期普遍缺乏。為了瞭解醫療資源缺乏對於東部地區6-18歲兒童、青少年口腔健康狀況之影響,因此進行此次調查。 目的: 探討台灣東部6-18歲兒童、青少年齲齒狀況與社會人口學、飲食習慣、口腔衛生習慣、就醫習慣之相關性。 方法: 本研究對象共768人,男生有384人(50%),女生有384人(50%)。採用分層多段隨機集束抽樣法,抽樣機率乃依據等比隨機抽樣(PPS)方式。問卷包括收集相關飲食、潔牙、看牙醫經驗資料及口腔健康狀況檢查。資料庫使用MS Access,資料分析使用JMP5.12軟體。 結果: 台灣東部6-18歲兒童、青少年之恆齒口腔健康狀況,在性別方面,男女生之DMFT index分別為2.45與3.43;齲齒率分別為30.65%與42.45%,12歲的DMFT index為2.41±2.41,恆齒齲齒率25.28%, 18歲的DMFT index為6.71±4.19,恆齒齲齒率66.52%,其齲齒狀況隨著年齡增加而有上升之趨勢。經由逐步迴歸分析結果顯示,性別、年齡、父母親教育程度與職業、飲食習慣、每天刷牙次數、定期口腔健康檢查等因素,與兒童、青少年的齲齒狀況有顯著性相關。 結論: 整體而言,台灣東部12歲學童的DMFT index為2.41顆,距離達到世界衛生組織(WHO)2010年DMFT index 少於2的目標仍需努力。在DMFT index與齲齒率方面,顯示男生優於女生,隨著年齡增加而增加;且與攝取愈多甜食、甜食後無潔牙習慣、每天刷牙次數、父母親教育程度等因素相關,所以加強兒童及青少年、父母親之口腔衛生保健教育,培養良好的口腔衛生行為和態度,方能提升兒童、青少年的口腔健康狀況。
Background: In eastern Taiwan living The special environment and the inconvenience of transportation, make visitins a doctor or dentist difficulf for residents in those areas. This has caused a lack of dentistry medical service resources for a long time. The aim of this paper was to study the affect of medical resources with 6-18 year-old children and their dental health condition. Aim: The purpose of this study was to investigate the association between dental caries status with dietary,dental hygiene habits and dentist visiting experience of children and adolescents aged 6-18 years old in eastern Taiwan. Methods: A total of 768 children and adolescents participated in this study. There were 384 males (50%)and 384 females(50%).They were sampled by stratified cluster sampling design using Probability Proportional to Size (PPS). A questionnaire including food intake, brushing teeth, dental visits experience and dental examination data was completed by all participants. A data-base was designed using MS Access and was analyzed using JMP 5.12. Results: The permanent dental health status of children and adolescents aged 6-18 years old in eastern Taiwan was as follows, the DMFT index was 2.45 for males and 3.43 for females; and the caries prevalence rates were 30.65% and 42.45% for males and females respectively, The DMFT index of children aged 12 years old was 2.41±2.41, with a caries prevalence rate of 25.28%; The DMFT index of children aged 18 years old was 6.71±4.19, with a caries prevalence rate of 66.52%;the dental caries status rose with the increase of age. Multiple regression analysis results showed significant related factors with dental caries status including gender,age, parents education, daily dietary habits, frequency of brushing teeth and regular dental visits. Conclusion: Overall, the DMFT index of children aged 12 years old in eastern Taiwan was 2.41, it is much higher than the goal of the WHO in 2010, that the DMFT index be less than 2. The male DMFT index and caries prevalence rates are better than females. it rose with the increase of age; and the related factors including intake of sweets、not brushing teeth after eating sweets、frequency of brushing teeth and the parents education . Therefore strengthening the oral hygiene health education for school children and parents, raising the standard of oral hygiene behavior and promoting the dental health condition of children and adolescents should be the focus of all dental care professionals.
