An Application of the Task-oriented Model in Stroke-Contemporary Concept

工作取向模式在腦中風病患之應用-現代觀念

摘要


隨著神經生理、生物力學、動作控制及學習等領域的發展,使我們對造成中風病人動作障礙的可能原因有了進一步的瞭解,也促使了新的中風治療模型-工作取向模式在近年內的興起。 此篇綜論性文章的主要目的即在介紹工作取向模型在中風病人之應用。本文共分三部份。第一部份討論近來有關構成中風病人動作問題之原因的研究報告。這些研究證據指出,肌肉骨骼系統的受限,如肌力減退及彈性結締組識的變化,和肌肉作用的空間、時間不恰當是其中的主因,而痙攣則被發現和動作障礙間沒有很清楚的關聯。因此,傳統上以中樞神經系統為評估及治療重心的神經誘發模式受到了挑戰。 在第二部份,本文即探討神經誘發模式的理論基礎。此模型以使中風病人動作正常化為治療目標。作者並列舉臨床工作者最為熟練的三種治療方法:本體神經肌肉誘發法(PNF),神經發展治療法(NDT)及布郎士壯氏的動作治療法(BRUNNSTROM'S MOVEMENT APPROACH)為例,討論其評估及治療原則。此模型之所以受到質疑的原因,除了過於重視與功能性動作無清楚關係的中樞神經系統損害,如不正常的肌肉張力、反射及痙孿,而忽略了其他周邊系統和環境的作用外,病患在治療中的角色是較被動的。基於以上的限制,以達成功能之任務為目標的工作取向模式即隨之發展而起。 本文第三部份即介紹以工作取向模式為基礎所衍生出的評估及治療原則。在評估上,我們應瞭解一個動作的產生是多重系統相互作用下的結果,而每個系統均處在平等的地位。因此在評估時我們應做全方位的考量,將中樞神經、骨骼肌肉等系統的損壞包括在評估之中。此外,評估的重點應在描述病患如何完成一功能性任務,及其使用之代償動作。而在治療上,我們應體認大部份中風病人終究仍有動作上之障礙,因此治療的重點應在教給病人最適合他現況的代償動作,並培養其適應環境變化的能力,以達成功能性的任務為治療目標。另外,動作學習的原則,如練習及回饋,也被應用在治療之中。文末作者並以「坐到站」之功能訓練為實例,加以說明工作取向模式的臨床應用。 身為臨床工作者,我們須瞭解沒有一個治療模式是絕對好或絕對壞的,如工作取向模式,由於強調病人的主動參與,治療師給予病患的感覺刺激減少了,另外有嚴重認知問題的病人也無法從治療中獲益太多,而如何定性或定量病人的代償動作也是對治療師的頗大挑戰。因此,無論是接受新的治療模式,或是修改傳統模式,或拮取各家之長,都應以使病人能得到最多的功能上之獨立為治療的終極目標。

Knowledge gained from neurophysiology, biomechanics, motor learning and motor control has provided us with further understanding about the causes of movement dysfunction in patients with stroke. Recent studies show that weakness, changes in passive properties, spatial and temporal abnormality of movement patterns play important roles in movement disorders in patients with stroke. However, spasticity appears not having clear correlation with the presence of abnormal movement. The traditional treatment approaches based on a facilitation model have therefore been criticized for overemphasis of central nervous system lesions, and underestimation of limitations from other systems. Because of the dissatisfaction of the facilitation model, a new model, the task-oriented model, is evolving. This model emphasizes on the accomplishment of functional tasks, the interaction of multiple systems, the importance of effective compensatory strategies and environmental adaptation. The principles of motor learning are also applied in clinical treatment. This paper reviewed the assumptions underlying the facilitation and task-oriented models, the assessment and treatment implications of these models and their limitations. The clinical application of task-oriented model was further demonstrated by using a functional task, sit to stand, as an example.
