摘要

Unilateral vocal fold paralysis (UVFP) can occur due to multifactorial reasons, which are associated with dysphonia, dysphagia, and aspiration. Due to the complexity of causes, there are no consistent treatment approaches for UVFP. In this study, we discussed a 57- year-old man who had left vocal cord paralysis after placement of the permcath catheter. After discussing the current issues, exploring the possible causes of unilateral vocal cord paralysis, we reviewed literatures to discuss the possible mechanisms and the latest treatment perspectives in this article.

單側聲帶麻痹有許多可能的原因，如手術導致、腫瘤導致、不明原因、神經性病變、插管導致、杓狀軟骨錯位、系統性疾病與藥物導致等等，而其會導致聲音沙啞、吞嚥障礙與吸入性肺炎等問題。目前單側聲帶麻痹有許多治療的方向與檢查方式，但至今因為檢查方式、處理的方式與討論到是否會恢復等問題，並未有一個很完全的定論。這次我們討論1名57歲男性，因雙迴路導管（permcath）置入後聲音沙啞5個月，就診發現左側聲帶麻痹，做了胸部電腦論層檢查後，實施自體脂肪注射治療，由他的治療經過與目前的問題做討論，來探討單側聲帶麻痹目前有的可能原因、治療做法［如語言治療、早期聲帶注射、喉返神經移植術（recurrent laryngeal nerve reinnervation）與甲狀軟骨成形術（Thyroplasty）等］與檢查方式［如喉部肌電圖（laryngealel ectromyography）、頸部超音波、胸部電腦斷層與頭部影像學檢查等］；也在這篇文章回顧近年重要的文獻，一同討論單側聲帶麻痹的可能機轉、處理流程與最新的治療觀點。
