護理人員專業承諾、工作滿意與離職傾向相關度性之研究 ~以某區域醫院為例

摘要


醫療院所工作的臨床護理人員是最大的照護人力資源,是病人的第一線照護需求者;更是所有醫療人員中與病人相處時間最長、關係最為密切的人。就目前的醫療環境,護理人員面臨許多的挑戰,加上健保系統對醫療機構的衝擊,間接影響護理人員的工作環境。近年來國內護理人員因工作辛苦、壓力大,且薪資報酬無法對等工作所付出之辛勞,以致護理人員的離職率不斷增加,造成護理人力大量流失,這不僅造成醫院的損失,更使得護理服務病患的品質之提升有著莫大的阻礙。 本研究旨在瞭解區域醫院護理人員之專業承諾、工作滿意度與離職傾向之現況,深入的及探討其關聯性。本研究以北區某區域醫院全體護理人員為研究對象,所得資料以SPSS 12.0中文版套裝軟體,進行資料分析。重要結論如下: 1.護理人員專業承諾整體平均得分為3.38分,工作滿意度整體平均得分為3.59分,護理人員離職傾向整體平均得分3.28分,屬於中等程度。 2.在護理人員對專業承諾與離職傾向的關係驗證結果顯示,護理專業承諾與離職傾向有顯著關聯性。 3.護理人員工作滿意度與離職傾向的關連,護理人員對直屬長官之「具有專業知能」、「隨時能指導部屬」、「頭腦清楚、條理分明」、「會鼓勵部屬進修」,獲得較高的滿意度。 4.整體專業承諾、工作滿意度與離職傾向,結果顯示「專業承諾」及「工作滿意度」有顯著相關。 5.依研究結果希望提供給護理行政管理者,重視護理人員專業承諾、工作滿意度等議題,以降低護理人員的流動率,提升照護品質,作為在護理人員的招募、培訓、留任之参考。

Clinical nursing staffs are direct caregivers, account for major manpower in hospitals; and the ones who take longest time and develop most intimate relations with patients among all paramedics. Under the circumstance of present medical environment, nursing staffs faced with many challenges, Moreover health insurance system's impact to Medical institutes, influence nursing staffs' working conditions indirectly. In recent years, domestic nursing staffs’ disproportionate laborious and high pressure work with salary reward leads to nursing staffs' unceasingly increase of withdrawal rate, thus nursing manpower lose massively causes not only the hospital’s loss, but also the greatest hindrance to promotion of the quality of care. This study aimed to understand present situation and discuss correlation of nursing staffs’ professional commitment, job satisfaction and inclination to quit of regional hospital. The findings of this study based on a survey of nursing staffs in a regional hospital were analyzed by SPSS 12.0 Chinese version coverall software , and crucial results are as follows: 1. The nursing staff professional commitment average score is 3.38, the work satisfaction average score is 3.59, the nursing staffs inclination to quit average score is 3.28. 2. There is remarkable correlation between profession commitment and inclination of quit. 3. On correlation between job satisfaction and inclination to quit, the nursing staffs have higher satisfaction to their immediate superior on “has professional knowledge and skill”, ” instruct the subordinate “,” has clear head, orderliness”, “will encourage the subordinate further education”. 4.The profession commitment, job satisfaction and Inclination to quit finally revealed that there is remarkable correlation between “profession commitment” and “the job satisfaction”. 5. Hope the findings of this study remind nursing administration superintendent of importance of the nursing staff professional commitment, job satisfaction to decrease nursing staffs' rate of turnover, promote quality of care, and as a text of recruiting, training, remaining.
