目的:探討老年人在實驗室進行心跳率控制訓練與進行自主心跳率控制訓練對心肺適能之影響。方法:以30名健康老年人 (年齡70.40±4.92歲、身高160.38±7.43公分、體重61.16±11.35公斤) 為受試對象,隨機分為訓練組 20名 (實驗室訓練組及自主訓練組各10名) 及控制組10名。以60%保留心跳率 (heart rate reserve, HRR) 到60% HRR減5下為心跳區間,進行8週,每週3次,每次30分鐘之心跳率控制訓練。訓練前後進行心肺適能測驗之前後測,心肺適能測驗項目為:6分鐘走路測驗、橢圓機IVHRC測驗及8分鐘橢圓機標準測驗。以單因子共變數分析三組間前後測心肺適能變項之差異性,本研究顯著水準α訂為 .05。結果:實驗室訓練組在橢圓機IVHRC測驗中進步了74.65%,顯著大於其他組,在8分鐘橢圓機標準測驗當中的生理指標 (VO2、VCO2、HR) 分別下降14.90%、17.91%、15.93%,顯著低於其他組。自主訓練組在6分鐘走路測驗中進步了7.25%,顯著優於其他組。結論:經過8週心跳率控制訓練後,訓練組的老年人的心肺適能皆比控制組顯著進步,然而實驗室訓練組在兩種橢圓機心肺適能測驗成績顯著優於自主訓練組,而自主訓練組之6分鐘走路測驗成績顯著優於實驗室訓練組。

English Abstract

Purpose: To investigate the cardiopulmonary effects by using supervised heart rate control training and home-based heart rate control training in elderly. Methods: The participants were 30 healthy elderly (age: 70.40±4.92 yrs, height: 160.38±7.43 cm, weight: 61.16±11.35 kg), equally assigned into 2 training groups (supervised and home-based) and 1 control group. The training groups were trained for 8 weeks, 3 sessions a week, and 30 minutes per session, in a heart rate control region between 60%HRR to (60%HRR-5) bpm. Cardiopulmonary assessment tests were made before and after the training, including a 6-minute walk test, IVHRC test and an 8-minute criterion test on the elliptical trainer. Data progressed by one-way ANCOVA to compare the cardiopulmonary difference between 3 groups. The significant level is set at α= .05. Results: The supervised training group enhanced 74.65% in the IVHRC test on the elliptical trainer, which is significantly greater than other groups, and the physiological response (VO2, VCO2, HR) in the 8-minute elliptical criterion test decreased 14.90%, 17.91%, 15.93%, which is significantly lower than other groups. The home-based group enhanced 7.25% in the 6-minute walk test, which is significantly greater than other groups. Conclusion: After an 8-week heart rate control training program, the training groups enhanced their cardiopulmonary fitness significantly greater than the control group; the supervised training group had better improvements in the cardiopulmonary assessment tests using elliptical trainer, on the other hand, the home-based training group has better improvement in the cardiopulmonary assessment test using the 6-minute walk test.
