糖尿病衛教對病患知識、態度、行為及代謝指標的影響

The Effects of Health Education on knowledge, Attitude and Practice in Seventy-four Type 2 Diabetic Patients

摘要

糖尿病是目前最重要的中老年疾病之一，其死亡率已佔國人之第五位，盛行率則不斷增加。糖尿病有很多急慢性併發症，急性併發症若不即時予以適當處理，可能造成後遺症，甚至死亡；而慢性併發症一旦發生以後常是不可逆的。這些併發症往往可藉良好的糖尿病控制得以事先預防或延緩其發生，而有效的衛教才能畢竟其功。本研究以門診第2型糖尿病患為對象，於87年10月至88年8月進行糖尿病人的衛教前後評估工作，衛教內容包括認識糖尿病，低血糖處理，藥物治療，運動和足部照護、心理衛生等，總計有74位糖尿病人完成全部衛教課程。經評估此74位病人糖尿病衛教前後知識、態度、行為和其代謝指標的改變，並予以量化統計，結果顯示衛教前後對糖尿病人知識、態度和行為有正向的增進，而糖化血色素值也有明顯降低；性別、年齡、教育程度和糖尿病史久暫等因素並未影響病人知識、態度、行為之增進，以及代謝指標之改善。建議未來除持續進行糖尿病的衛教工作，並能同時以不同的衛教方式進行，使維持和改善糖尿病人的生活品質及預後。

Diabetes mellitus is one of the most important chronic diseases and is the fifth leading cause of death in Taiwan. The complications of diabetes can be prevented, halted or delayed by means of good glycemic control according to the Diabetes Control and Complication Trial (OCCT, 1993) and the United Kingdom Prospective Diabetes Study (UKPDS, 1998) Diet modification, regular exercise and medications, if necessary, are essential for diabetic management. However, good glycemic control can never be achieved without health education. In this study, we evaluate the effects of diabetes health education on knowledge, attitude and practice of diabetics. From October 1998 to August 1999, seventy-four outpatient type 2 diabetic subjects were enrolled. Assessment of knowledge attitude and practice of these patients were performed before and after four courses of health education. . The results showed positive changes in all the three items. The effects of diabetes education are independent on the age, sex, duration of diabetes and school-education of the diabetics. We conclude that the health education is important, effective and should be provided to all the diabetic patients.
