Delayed Deep Femoral Artery Injury after Austin-Moore Hemiarthroplasty Complicated with an Intra-operative Periprosthetic Fracture: Report of a Case

摘要


髖關節置換術後發生遲發性血管傷害的案例非常少見,而發生的原因大部分都是假性血管瘤,術前即已存在的骨碎片,或是術後外傷所造成。本文提出一個從未被報導過的案例:一位85歲的先生因爲股骨頸骨折接受人工髖關節置換術,在術後47天發生深股動脈穿刺傷,造成血管傷害的原因是一個骨碎片,而這骨碎片是在人工髖關節置換術中發生的。我們實施血塊清除,骨塊移除,以及血管修復手術。這位老先生術後復原情況良好,並在一年後已能靠枴杖的幫助行走。

Delayed vascular injuries after hip arthroplasty are very rare, and most of the causes were false aneurysm, preoperative bone fragments, or postoperative trauma. We report a case of deep femoral artery injury 47 days after Austin-Moore hemiarthroplasty for an 85-year-old male with femoral neck fracture. The cause of arterial injury was an iatrogenic bone fragment during hemiarthroplasty, which has not been reported. Hematoma evacuation, removal of the bone fragment, and vascular repair were performed. The postoperative course was uneventful and the patient could ambulate with the aid of a walker at 1-year follow up.
