甲狀腺超音波自費健檢

Thyroid Ultrasonography in Self-financed Health Checkups

摘要


背景:甲狀腺癌發生率在過去30年不斷上升,跟檢查技術及甲狀腺超音波的進步有關。隨著健檢醫療的發展,我們嘗試瞭解甲狀腺超音波健檢的面貌。方法:回顧2013年1月到2017年11月,所有在羅東博愛醫院接受甲狀腺超音波健康檢查的病患,總共有2,294名。分析其基本資料及甲狀腺超音波健檢的結果。結果:2,294名病患中,女性佔37.6%,平均年齡47.3歲。超音波下有甲狀腺結節的比例為38.0%。女性有甲狀腺結節的比例(43.6%)顯著高於男性(36.6%);有結節之病患平均年齡較大,達顯著差異。246名病患(10.7%)有單一甲狀腺結節,根據結節的超音波表現,高危險的單一結節佔其中的5.2%。我們對2,294名病患中的59名,進行了共76次的細針穿刺檢查。最終篩檢出甲狀腺癌的病患有4名,佔所有健檢病人的0.17%。4名都是乳突性甲狀腺癌,後續接受手術治療及追加放射性碘治療。此外,我們還發現16名(0.69%)自體免疫甲狀腺疾患。41名甲狀腺機能亢進,22名甲狀腺機能低下,以及1名副甲狀腺腺瘤。結論:甲狀腺超音波健檢不但可以篩出甲狀腺癌,同時也篩檢出其他的甲狀腺疾患。對這些特别的族群是很有價值的診斷工具。

BACKGROUND: An increasing incidence of thyroid cancer has been observed over the last three decades. This is related to developments in skills and equipment for thyroid ultrasonography. Due to advances in self-financed health checkup, we tried to reveal the picture of the self-financed thyroid screening and ultrasonography. METHODS: We reviewed the data of all patients who attended Lotung Po-Hai Hospital for self-financed thyroid ultrasonography from January 2013 to November 2017. We then analyzed the general data and the results of self-financed thyroid ultrasonography. RESULTS: Of the 2,294 patients included in the series, 37.6% were women. The mean age of participants was 47.3 years. Of the patients, 38.0% had thyroid nodules determined through ultrasonographic examination. Thyroid nodules were significantly more prevalent in women. Also, patients with advanced age were more likely to have thyroid nodules. Of the patients, 246 patients (10.7%) had solitary thyroid nodules, and according to ultrasonographic patterns, 5.2% of patients were at a high risk of developing thyroid cancer. We performed fine-needle aspirations 76 times in 59 patients. We revealed that 4 patients had thyroid cancers in the series, account for 0.17% of all patients. All had papillary thyroid carcinoma. All 4 received surgical treatment and 1 received adjuvant radioiodine ablation. Furthermore, we discovered that 16 patients (0.69%) had autoimmune thyroid diseases, 41 patients had hyperthyroidism, 22 patients had hypothyroidism, and 1 had parathyroid adenoma. CONCLUSION: Thyroid ultrasonography screens for thyroid cancers and other thyroid disorders. Thyroid ultrasonography was a valuable diagnostic tool for these selected patients.
