健康科技評估的倫理考量與挑戰
The Ethical Consideration and Challenge of Health Technology Assessment (HTA)
摘要
健康科技評估(Health Technology Assessment以下簡稱HTA)之目的在於提供各種不同層面的健康科技考量，舉凡醫療政策制定、醫療機構規範實施及個人醫療決策選擇，都可藉由健康科技評估的協助，以做出是否採取某特定健康科技(health technology, HT)的決定，而其中倫理面向的考量則是HTA中極重要的一環。本文針對HTA的倫理面向作一介紹與反思，包括HTA與倫理之關係、HTA過程所可能產生的倫理議題、進行倫理評估的方法、檢視HTA報告之方式、及倫理評估應用上之困難等，並以人工電子耳案例來闡述倫理評估在HTA中的重要性與挑戰，最後介紹HTA在台灣之發展與現況。本文期望透過對HTA倫理面向之系統分析，闡明倫理考量在HTA中的重要性與應用方法，並建議將來國家相關機構於進行健康科技評估時，能重視並有系統地進行倫理面向的分析與評估，使HTA能提供更周全之決策參考。

This paper provides a systemic explication and reflection of the ethical domains in Health Technology Assessment (HTA), which include the relationship between ethics and HTA, the ethical issues of HTA, the theories and approaches for HTA ethical analysis, the methods for examining HTA report, and the difficulties in performing HTA ethical assessment. A case study of ＂cochlear implant＂ is adopted to demonstrate the importance and challenge of ethical assessment in HTA. Finally, the past development and current status of HTA in Taiwan are explored and analyzed. The authors suggest that promoting more systematic and comprehensive ethical assessment in HTA is an important trend and challenge to be achieved in Taiwan.
