摘要
本篇個案報告是敘述一位68歲女性腦中風病患，因左大腦血管梗塞導致右側肢體偏癱，面臨疾病所帶來身體失能之情況。筆者於病患住院期間，運用觀察、會談等技巧收集病史，並以Gordon十一項功能性健康模式為評估指引，協助病患增進對疾病及肢體復健之認知，透過知識教育減輕焦慮，護理期間自2008年1月12日至2008年2月5日，經內容分析發現，個案在住院期間所呈現的護理問題有：1.身體活動功能障礙、2.自我照顧能力缺失、3.焦慮、4.特定的知識缺失。於護理過程中，藉由與個案和家屬的良好互動及會談，並提供正向的支援、鼓勵與陪伴，引導個案接受身、心復健治療，增加個案對治療計劃的配合度，及早接受病情變化，減輕對疾病的焦慮。

This case report is a 68 year old female suffering from left brain infarction with right hemiplegia and facing the physical dysfunction. We try to collect the information about her present and past history by means of observation and interview during hospitalization. We help her with the perception of disease and rehabilitation and reduce her anxiety by education. Gordon's 11 Functional Health Patterns was administered for the indication of evaluation during nursing from January 12th, 2008 to February 5th, 2008. In analysis, it was identified that the patient had the nursing problems of: 1. physical function impairment, 2. lack of self-care ability, 3. anxiety, and 4. deficiency in specific knowledge. In the nursing process, we made good interaction and interview with the patient and her family, and provide positive support and encouragement. We aimed to help her accept the disease and physical and psychological rehabilitation, reduce the anxiety about the suffering, and cooperate in the program of treatment and therapy.
